Suggests iron deficiency and combined with above results looks like iron deficiency anaemia. Speak to doctor who should really do an iron panel to confirm iron deficiency and should prescribe iron tablets, take each one with some Vit C to help absorption and help prevent constipation. Make sure you levels are monitored regularly, should be every 2-3 months, iron panel is important to monitor to ensure serum iron and saturation percentage dont go over range.
B12 is good. Do you supplement?
Folate should have been tested as well as B12 and folate work together.

Both FT4 and FT3 are low in range if you take only Levo.
If taking combo meds then FT4 might be OK for you but FT3 is low. What thyroid meds are you taking, I'm guessing with such a low TSH you're taking Levo plus T3 or NDT.
Yes, I’m taking 100 mcg thyroxine and 10 mcg of Liothyronine in two 5mcg doses. I feel exhausted, I’m constantly sweating have gained lots of weight and just want to sleep all day. I’m assuming where is says Bone Profile these are the Vitamin D tests? The results are below.
Serum Calcium is 2.32 range 0.232 - 2.784
Adjusted Calcium is 2.35 range 2.2 - 2.68
Serum Inorganic Phosphate is 1.1 range 0.77 - 1.61
I had a bone density scan after I broke my wrist and was found to be at risk and was put on Vitamin D tablets which I take 2 tablets twice a day. I have been taking these for over a year.
OK, so folate is very low, it's recommended to be at least half way through range which would be 10 plus with that range. Consider taking a B Complex to improve your level.

The Vit D test shows as 25-Hydroxyvitamin D or similar. What you have above is a bone profile which doesn't include Vit D, it has to be ordered as a separate test and many GPs are unwilling to do this.

The Haemoglobin is quite substantially below the range indicating that you are anaemic. Since your Ferritin (iron stores) is also substantially below the range it is likely that your anaemia is caused by iron deficiency. Your low RBC (Red Blood Cell Count) will probably be a result of your anaemia as well and should improve when your iron levels start to improve.
Your doctor should prescribe iron supplements (iron salts) for you. Please note that there are many kinds of iron supplements and are not all alike. You might find the ones your doctor prescribes to be poorly tolerated. Some alternatives to the prescribed iron salts are discussed in the following two links. They are not prescribed and would have to be bought online. Doctors won't prescribe them.
You could ask your doctor to give you a full iron panel and another Full Blood Count after 2 - 3 months of supplementing just to be sure that you are absorbing something from your supplements. Be aware that improving iron can take many months - don't expect everything to be fine in the first three months.
Your anaemia, low ferritin, and low RBC will be causing lots of fatigue and breathlessness.
Your B12 is above range - this is fine. B12 is not poisonous.
In order for the body to make use of the B12 you have you need adequate levels of folate too. If you haven't had this tested then it is a good idea to ask your doctor to test it for you.
The best folate supplement for many people is methylfolate. Avoid folic acid, which is what doctors usually prescribe. Anyone with a (very common) MTHFR gene problem can't absorb folic acid very well and they end up with high levels of unmetabolised folic acid in their bloodstream which is not a good state to be in. Methylfolate can be used by the body without issue by most people with or without MTHFR problems. In people who are very low in folate there is a possibility they will get side effects from it.
