how my appointment went -with results

I saw a professor who is on the ‘list’ he performed lots of tests.

MRI all normal.

Thyroid bloods in my profile from Nov

jan TSH 0.58 and FT4 11.5 same ranges as previous.

we went back to through my bloods since 2017 it seems my TSH and FT4 always waiver between 0.35-1 and FT4 no more than 12.

B12 458 (197-771)

Serum folate 13.9 (>3)

Antinuclear abs negative

Creatine Kinase 52 (25-200)

LDH 163 (135-214)

C-reactive protein <1 (0.1-5)

Serum cortisol 348 taken at 12.30ish

Prolactin 322 (102-496)

I can’t find my ferritin but I know I have posted it on here before and comments said it was normal.

Vitamin d not done but I take a high dose daily of this so I suspect fine.

He suggests my symptoms are not thyroid related or pituitary. And low level hormones are my norm. I’ve only been feeling symptomatic since June time.

So I guess I need have other investigations.

no, I don’t think both antibodies were done.

Good shout about the thyroid.

I stopped multivitamin but taking vitamin d. Still take ashwaganhha.

I kind of feel there’s not much else I can do I’ve been seen by an excellent and recommended consultant and he thinks everything is ok and he said he tried everything to find something wrong.

Suggest you get full thyroid test including BOTH antibodies 6-8 weeks after stopping multivitamin

Test early Monday or Tuesday morning, ideally just before 9am

Thanks. Can I ask why 6-8 weeks after stopping multivitamins? It shouldn’t take that long for them to leave your system? Why before 9am? I want my TSH and FT4 at its lowest. Thanks

You want TSH at highest and Ft4 at lowest

Multivitamins contain iodine

Iodine affects thyroid

Nothing in Thyroid world changes quickly, ESPECIALLY TSH

It’s always advisable to wait at least 6-8 weeks after any change before testing

why on earth do you want TSH at lowest

You need to test to get TSH at its highest

as I have low TSH but not below range, the HCP alluded that in order to be treated by TSH has to be below range or above

Below range is usually for diagnosing hyperthyroid

Email endocrinologist and ask him

Your TSH is already low enough combined with very low Ft4 as well for central hypothyroidism

thanks but my endocrinologist doesn’t agree. Apparently I need more than low TSH and FT4 to have SH. Below range, would need treatment or another marker like prolactin, FSH or LH. My LH has only been below range once but otherwise in range and also my FSH is in range. My TSH and FT4 have been low since 2017 so endo has said low range is my normal.

Anything significant happen in 2017 or was that just first thyroid test

And why was thyroid being tested so regularly

Do you have hypo symptoms and/or low vitamin levels

Low oestrogen, progesterone or testosterone

Assuming you disagree with endo and want to continue to pursue getting a diagnosis

Yes ….test thyroid after lunch for lowest TSH

Consider Ultrasound scan of thyroid and definitely get both thyroid antibodies tested

Also get full iron panel test for anaemia 6-8 weeks after stopping multivitamin

High ferritin can be misleading. You can have high ferritin (due to inflammation) and low iron

Test early morning and fast.

Don’t eat red meat night before test

thank you. Yes low oestrogen, not tested progesterone or testosterone but going to.

2017 just exhaustion. I have 3 children though. This year I went through a tough time and was anxious so they put it all down to anxiety since then.

Hypo symptoms but this can cross over with Peri menopause which I’m now on HRT due to symptoms and low oestrogen.

I’d like to trial Levo to see if it manages my symptoms, but the endo (recommend by thyroid UK) couldn’t find anything wrong on all test and MRI other than low TSH and FT4. So will not treat.

Other than retesting I don’t think I can do anything as I have seen a professor in endo who will not treat and also a GP who will not treat if consultant won’t treat

Different endocrinologists have different ideas

Many thyroid patients have seen several different endocrinologists

Suggest you gather up more test results first

Ultrasound scan

Full thyroid and vitamin testing 8 weeks after stopping multivitamin and iron panel test

Cortisol and Dhea test via Regenerus

Are you overweight or underweight

What about your fitness?

Finding time for some exercise/yoga/ swimming etc

I’m in the normal range for weight. I exercise daily, either walking, swimming, running etc.

I eat a very healthy diet.

Is it just the multivitamin I need to stop? I can continue with cod liver oil and vitamin d?

yes

stop multivitamin …..wait 6-8 weeks and then test FULL thyroid including both thyroid antibodies and vitamin levels

See what results show

Then you may need to add separate vitamin B complex if folate/B12 are low

People vary widely in how much vitamin D they need

Did you test before you started?

Recommended NHS dose is 400iu, which is very conservative

Vitamin D is oil based so we can’t get rid of excess. Should test before starting and retest twice year, especially initially to work out how much you personally need

Cod liver oil also contains vitamin D

How much vitamin D Approx in cod liver oil dose you’re taking

Test first

No one in U.K. gets any significant vitamin D between Oct and April

Test twice yearly

Once via NHS private testing service
