Had my latest results from MMH and my Tsh has gone up and t3 and t4 down August I was told to drop down to 100mgs of thyroxine was previously on 125mg one day and 150 the next day so am confused now August Tsh was 1.81 range 0.27 to 4..2 T4 was 22.5 range 12 to 22 and T3 was 4.3 range 3.1 to 6.8

Now Tsh is 3.03 range 0.27 to 4.2

T4 is 20.5 range 12 to 22

T3 is 4 range 3.1 to 6.8

Any advice appreciated

Levothyroxine was reduced 6 weeks ago down to 100mg and still same brand

Added Nhs results from yesterday

Thyroid function test

Serum TSH level 5.2 mIU/L [0.3 - 5.5]

Serum free T4 level 20 pmol/L [10.0 - 22.0]

How long exactly since dose levothyroxine was reduced

Are you still taking same brand of levothyroxine

Previous posts says you were taking 125mcg and 150mcg alternate days (not 175mcg)

Previous results

TSH 1.2 range 0.3 to 5.5

Ft4 28 range 12 to 22

Ft3 3.9 3.9 to 6.8

Test done at 8.30 no food or drink before and no thyroxine 24 hrs before

currently on 125mgs one day 150mgs next day

no biotin 2 weeks before and all other vitamins were normal.

difficult to read results above

Looks like

TSH 2

Ft4 20.8 (12-22)

Ft3 4 (3.1-6.8)

Is that correct results?

Have amended to say levo reduced to 100mgs 6 weeks ago down to 100mg on same brand now as before test done before 8.30 no food or drink and no biotin etc 3 weeks before test

FT4: 20.8 pmol/l (Range 12 - 22)

Ft4 is 88.00% through range

FT3: 4 pmol/l (Range 3.1 - 6.8)

Ft3 only 24.32% through range

shows very poor conversion rate of Ft4 to Ft3

What vitamin supplements are you currently taking

When were vitamin D, folate, ferritin and B12 last tested

You could try splitting your dose levothyroxine

Taking half dose waking up and half dose at bedtime

Might improve conversion rate

Retest thyroid again in another 8 weeks

If Ft3 not improving….going to need addition of T3 prescribed alongside levothyroxine

Done on the 15th September am on 5 mgs folic acid and 210mgs of ferrous fumerate

Is ferritin really only 6.7!?

optimal ferritin is at least over 70 and around 100 likely better

Look at increasing iron rich foods in diet 

Eating iron rich foods like liver or liver pate once a week plus other red meat, pumpkin seeds and dark chocolate, plus daily orange juice or other vitamin C rich drink can help improve iron absorption

This is interesting because I have noticed that many patients with Hashimoto’s disease and hypothyroidism, start to feel worse when their ferritin drops below 80 and usually there is hair loss when it drops below 50.

Never supplement iron without doing full iron panel test for anaemia first and retest 3-4 times a year if self supplementing. It’s possible to have low ferritin but high iron

so ferritin is deficient

Folate inadequate

supplementing a good quality daily vitamin B complex, one with folate in (not folic acid) 

This can help keep all B vitamins in balance and will help improve B12 levels too

B vitamins best taken after breakfast

Thorne Basic B recommended vitamin B complex that contains folate, but they are large capsules. (You can tip powder out if can’t swallow capsule) 

IMPORTANT......If you are taking vitamin B complex, or any supplements containing biotin, remember to stop these 7 days before ALL BLOOD TESTS , as biotin can falsely affect test results

In week before blood test, when you stop vitamin B complex, you might want to consider taking a separate methyl folate supplement and separate B12

With serum B12 result below 500, (Or active B12 below 70) recommended to be taking a B12 supplement as well as a B Complex (to balance all the B vitamins) initially for first 2-4 months.

once your serum B12 is over 500 (or Active B12 level has reached 70), unless vegetarian or vegan stop the B12 and just carry on with the B Complex.

if vegetarian or vegan likely to need to carry on separate B12 supplements….unless on B12 injections

Ferritin is deficient

Have you had full iron panel test for anaemia…if not that’s next step

If not request/insist GP do so or test via Medichecks

High TIBC means that you have low levels of iron. When there isn’t much iron to attach to, you’ll have a lot of free transferrin in your blood.

Low iron levels can lead to iron deficiency anemia, a condition in which your body doesn’t have enough iron to make healthy red blood cells.

This can be caused by:

Blood loss

Vegetarian or vegan diet

Coeliac disease

Very low transferrin saturation

What does it mean if your Transferrin saturation (Iron Saturation) result is too low?

Low transferrin saturation indicates you are likely to be suffering from iron deficiency.

In iron deficiency, iron is low and fewer transferrin binding sites are used to the saturation of these binding sites is low.

Causes of iron deficiency can include:-

Insufficient dietary iron intake

Inadequate iron absorption in the stomach

And extremely low ferritin

Has GP prescribed iron supplements

gp has put me on ferrous funerate 210mg and advised me to take 5mg folic acid

Rather than folic acid….suggest you take daily vitamin B complex as detailed in my reply above 2 days ago

Low iron/ferritin takes many months to improve

Frequently NHS stop supplements too soon and/or if not increased iron rich foods in your diet, levels just drop once you stop

Was test done early morning, ideally before 9am and last dose levothyroxine 24 hours before test

ferritin Level 30ug/L range 13.0 to 150.0

B12 526 ng/L range 197.0 to 771.0

Folate level 5.7 ug/L range 2.1 to 26.8

Are you now taking daily vitamin B complex for low folate

You need vitamin D tested

And full iron panel test for anaemia

Privately if GP won’t
