You will need to add the reference ranges for your test so that members can interpret them.
Your TSH looks very high for that level of FT4. Could anything have skewed the result? Did you take Biotin or B Complex or hair/nail supplement in the week before the test? Did you drink coffee or eat before the test?
Sorry don't have the reference ranges, they are from GP tests, results on my GP. Did not take any supplements for almost 2 weeks. Did not eat or drink before test

So the app isn't giving you ranges? At least we know that TSH is definitely too high. T4 looks ok / high in range, but again, impossible to say without ranges. Is it total T4 or Free T4? Will be useful to see what FT3 gives. Also can't say much about PTH without the range.
Are you still on 125 mcg levo? Brands?
yes still 125 mcg 100mcg is Accord 25mcg Northstar (Teva).
Everything you wrote to slowdragon below looks ok at first glance (new results eg iron). Depending on what Ft3 shows - but your last one was miserably low - I think your next step might be to do a trial of T3. See what the admins say.
And definitely go back on your regular B12 jabs As often as you need them - be that daily or every other day (eod) or weekly or whatever. Don't forget folate. Your result doesn't look low but it's not super high either. You might want to add in at least 400mcg folic acid per day if you go back to daily/eod (I couldn't find back what your SI frequency was).
I had been trying to inject ever other day but sometimes fuddled head forgot but usually at least twice a week. I injected as soon as I got in from bloods appointment, again today, should I get a B complex or just folate , I certainly felt the difference when not injecting

Yes a B complex is always a good idea! Just make sure it doesn't have more than 10mg B6 in it, as it can build up and become toxic. I personally like the "Nature Made" brand B complex but I don't know if you can get it in the UK. It doesn't have folic acid in it, which is fine for me because I take it separately. I still need 5mg folic acid per day but I guess most people don't. Trial and error, see what works

yes sample for Blue horizon was 9th May. Vit D was requested but no result as yet. B12 now 2000. ng/L no ref range, folate now 12.6 ug/L , no ref range, Ferritin 117.0 ug/L no ref range, 25.0umol/L serum iron no ref range, 2.93 g/L serum transferrin, 74.0 umol/L unsat iron binding capacity, 34.0% transf. saturation all no ref ranges FT3 was requested no result yet. same 125mcg, took no supplements since about 10th or 11th, no B12 injections,

I've put your blood results for TSH, FT4 and FT3 through the SPINA-Thyr algorithm.
I've done so on the basis that you only medicate with T4, and have no underlying conditions which might render the results useless.

As you probably already know, you are a poor converter, and its very bad at 16.72 pmols. A healthy thyroid person would be converting at around the 33 pmols. According to the 2015 paper by Dr Midgley under 23pmols is a poor converter.
Please do read through the document I've linked, and use the calculator as I've done. What is most striking is that your TTSI is 696. TTSI looks at resistance to thyroid hormone. 100-150 is considered normal level, and 308 is mild. A level of 747 is considered severe.
I have undiagnosed resistance to thyroid hormone, and your blood results caught my eye.
A recent paper suggested that the FT3 reference range for RTH may be as high as 8.0 - 8.6 although it could be higher or lower.
I am not medically trained at all, just someone who's been struggling all my life with this.
