Help please with latest results T3/T4 combo

Hello everyone

Thank you for all your help and advice to date. I’ve recently had a full thyroid panel and am now 3 months into a trial of T3.

My new results as follows:

TSH 1.29 (0.27-4.2)

T4 15.4 (12-22)

T3 4.11 (3.1-6.8)

My dose as follows: 100mcg 3 days and 75 for 4 days Levo

T3 15mcg in 3 divided doses 6am, 2pm & 10pm

I was originally on 100mcg per day before introducing T3 when my Endo reduced Levo to 75mcg each day. It was said on here that likely I didn’t need the initial reduction, and after an initial 5 to 6 week period of improvement when introducing the T3, I have since felt undermedicated and quite unwell again.

From my previous bloods, the T4 has increased, but this is still only about 30% through range, as is my T3. I had hoped to see an increase in T3 with adding Lio, but it remains stubbornly at the 4.2 mark. I can’t seem to get beyond that level since diagnosis 2 years ago.

Bloods have been done according to protocol and vitamins are in the top third of range so I am only supplementing vitamin D (Better You spray with K2). I’m planning to retest in 2 weeks time, along with iron panel as my last post referred to ‘unusual’ results when last tested. (A huge thank you for the input on those!)

My Endo has suggested a further day of 100mcg (4 in total and 75mcg x 3).

I had wanted to trial changing Levo to night time dosing, but am inhibited as am on an antidepressant which needs to be taken at bedtime so believe this rules this out.

I’m wondering if I should request the dose be increased a little more, given how fatigued/ unwell I’m feeling?

I’m going for an SST test this week to check/ rule out adrenal insufficiency.

In my opinion you should go to 100mcg every day, I honestly don't think one extra day of 100mcg instead of 75 will help, that's just an extra 25mcg per week.

Considering your results on 100mcg Levo only daily were

TSH 2.47 (0.27-4.2)

T3 3.22 (3.1-6.8)

T4 15.9 (12-22)

as I said before in reply to your other thread there was no need to reduce your Levo when adding T3 as your FT4 was only 39% through range

Thank you SeasideSusie I think I need to as whilst I’m all for small increases, I feel I’m languishing in illness and there would be no danger of over medication on these results. I remember you saying about the reduction and think you are absolutely right. I can see the Endo wanting to be cautious but I do need to see and feel some improvements now.

Your FT3 is incredibly low for someone taking 15 mcg T3. Do you always take it on an empty stomach, two hours after eating or one hour before, just like levo?

Thank you for responding. Yes, I’ve been quite regimental in the last 6 weeks (the odd one a bit too close to food maybe) so it is a bit perplexing! I just can’t work it out.

Looks like an absorption problem. Leaky gut, perhaps, or low stomach acid.

I’m almost 6 months in on GF and was debating stopping as I’ve not noticed a huge difference, but have had a more quiet stomach. I’ll continue and look up other things that may help. This is really helpful thank you 😊

If your problem is low stomach acid, GF is not going to help with that. What people often do is start eating gluten again, and see if it has any adverse effects. If it doesn't, then there's no point in continuing with GF. But, if it does, then they go back to being GF.

Ok I’ll give this a try! I’ve been worried about changing anything but I do find it a bit restrictive at times. Nothing that can’t be managed, but if it isn’t helping then no point in continuing. As you say, if I’m worse, can always switch back. Thanks greygoose for helping. It can be a difficult road to navigate so advice is very much appreciated x
