Yup that's pretty bad conversion! Hopefully you'll get a trial of lio (you won't get NDT prescribed if you're in the UK).
In the appointment itself I'd recommend you get across how - despite excellent nutrients and slightly over-range free T4 you just don't feel properly well. If you're allowed to take someone with you who can reinforce that - so much the better. Have a specific list of WHY you aren't tip-top and that the answer MUST be the too-low free T3.
Then, if you get that prescription (crossing fingers) - remember to take it "low and slow" to start with, You are likely to have levo reduced so do that first. Then a week later, introduce 1/4 tablet of lio with your levo, A week after that increase to 2 x 1/4 tablet (taken apart) etc until you are on a whole tablet, in two or more doses, or you feel properly well, and work out if you feel best in 1, 2 or 3 doses. Hold and re-test TSH, free T3 and free T4. Good luck x
Yes ditto the previous post : and put another way if you need to say something else :
I believe the accepted conversion ratio when on T4 only is 1 - 3.50 / 4.50 - T3/T4 - with
most people feeling at their best when coming in at around 4 or under :
Your conversion is coming in at 6.76 showing very poor conversion and the logical solution is to introduce in a little T3 alongside a slightly reduced dose of T4 .
To find your conversion ratio you simply divide your T3 into your T4 :
Your TSH is in range so hopefully you will be luckier than I was - Good Luck - though it shouldn't even be in doubt, and down to fingers crossed, or tea leaves in the bottom of a cup.
Read up about conversion problems if you have the DIO2 gene. It looks likely in your case. Mention this to the Endo and watch his eyes glaze over. He may not have much knowledge about this, but it will show him you are taking your health seriously and have read up on it, and may get him to consider T3. Good luck.
