Help with blood results with 125 mcg of T4

Hello.

Male, 42 years old, 73 kg, on 125 mcg of Synthroid.

Did my blood test after 6 weeks on that dose. Blood was taking in the morning after 24 hours of last dose, and here are the results:

TSH: 0.56 REF: 0.35 – 4,94 uiU/ml

Free T4: 1.02 ng/dl REF: 0.70 – 1.48

Free T3: 2.74 pg/ml REF: 1.88 – 3.18 (the old reference range from this same laboratory was 1.81 – 4.59 pg/ml, I don’t know why the upper limit is to low now.)

I still have symptoms of stiff knees joint, hair loss, my face is sagging whit horrible nasolabial folds that was not here before I got hypo, etc.

Should I go for the 137 mcg Synthroid?

I tried T3, but even 2.5 mcg of synthetic T3 makes me hyperthyroid, whit extreme exhaustion, irritability and anxious. I tried to ¼ of NDT and I feel the same, I feel worse whit T3, it looks that I don’t tolerate it.

My b12, ferritin, folate and vit d3 are fine.

Is someone here to have problems with t3 even at a small dosage?

Also, my SHBG is always high, is it related to thyroid disorders or not?

I think it's worth trying 137.5mcg. Your numbers look quite good, but there's room for them to move up in the range.

Looking at previous posts, it seems you probably have Ord’s thyroiditis

Are you on strictly gluten free diet or tried it?

Doctor said that my thyroid gland atrophied because of the Synthroid dose, and If I come off thyroid medication she will come back to the natural size for a male.

Without converting your results to the same units as typically seen in U.K. remember that lab ranges are not healthy/not-healthy ranges your healthy range will be very narrow in comparison and probably around your peer group normal healthy values I.e. without thyroid disorder. That said some of your symptoms may be from iatrogenic testosterone deficiency caused by excessive SHBG associated with thyroid treatment, you could try a boron supplement to ameliorate that and reduce thyroid treatment to the least possible in line with ameliorated hypothyroid symptoms and reasonable blood hormone levels. You could also try split dosing thyroid replacement to avoid flooding the liver with thyroid which is what May be causing high SHBG which mops up free testosterone so even higher total testosterone may not be doing much!

D3: 61,3 ng/ml ref 20 – 100

Estradiol (E2) 30,94 pg/ml ref: 11,30 – 43,20

Ferritin: 159,35 ng/ml ref: 21,81 – 274

SHBG: 55,48 nmol/l ref: 18,30 – 54,10

Free Testosterone:12,48 ng/dl ref: 5,70 – 17,83

Testosterone: 720 ng/dl ref: 175 – 781

So, is the high SHBG that is causing my hypothyroidism?

Is hypothyroidism related to low SHBG?

My free t4 in pmol are 13.2 and free T3 in pmol are 4.2.
