Should I increase my dose? I am male, 41 years old, 74 kg, and I am taking Synthroid and Desiccated Thyroid.
I take 100 mcg of Synthroid in the morning 1 hour before breakfast whit half a grain of Desiccated thyroid and before lunch I take another half grain, so total is 138 mcg of T4 and 9 mcg of T3.
Whit 8 weeks of this dose, my results are as follow:
TSH: 0,02 uIU/ml Ref: 0,35 – 4,94
Free T3: 0,36 ng/dl Ref: 0,20 – 0,44
Free T4: 1,09 ng/dl Ref: 0,70 – 1,48
So, my Free T4: Free T3 ratio is 3:1, so it looks that my conversion is fine.
Also, my Free T4 is at 50% in the reference range and Free T3 at 70% on his range.
I tried only desiccated thyroid, going up to 2.5 grains, but don’t feel any difference and the labs showed that whit only ndt I get to low free T4 and higher, out of range Free T3.
So, it looks that my labs are perfect now.
But, I still have all the symptoms of hypo!! My eyebrows are missing, I have brain fog, I wake up at 35.9 Celsius and never reach 37 Celsius, and my most depressing symptom is the joint pain, heaviness and weakness on my legs!! They felt rusty!!
Maybe I don’t had hypothyroidism, maybe my adrenals are the true cause. I will ask my doctor a trial on hydrocortisone, and if the pain on my legs go away, it will show that the root cause is the adrenals.

So, my Free T4: Free T3 ratio is 3:1, so it looks that my conversion is fine.
Conversion doesn't come into it when taking T3 in any form, including NDT. You are taking T3 exogenously so it's not possible to know what the natural conversion is, that can only be seen when taking Levo only.
Your adrenals may be a problem. Your vitamin and mineral levels may be a problem.
Have you had Vit D, B12, Folate and Ferritin tested?
I forget to ad that whit this 100 mcg of T4 and one grain of NDT my Total Cholesterol reduced from 290 to 195, so maybe the thyroid treatment is doing something at the cellular level.
My 8:00 am serum blood cortisol level was always good, at around 16-18, whit the top reference range of 22-25. But, you know, the internet say that the salivary cortisol test is better than the blood.

The old reference range is irrelevant. The range has obviously changed, as they can do, so you can only take the range that comes with your result.
We're all different as to where we need our levels of FT4 and FT3 to be so all you can do is experiment and see where you feel best, ensure your nutrient levels are optimal, and if you feel that it's necessary then do an adrenal test.
It is stated that 1 grain of NDT, is equal in its effect, to 100mcg of levothyroxine. One professional I saw, stated that we should take care of the adrenals as they were probably suffering but another stated that the adrenals will take care of themselves once on an optimum dose.
