Hashimotos normal TSH?

Firstly apologies for yet another post! I’m new to all this and getting myself into a bit of a tailspin which I know isn’t helpful.

Brief history I’ve been unwell since last Nov with no diagnosis. Ultrasound detected 2 ‘lumps’ on thyroid recently. Advised not suspicious but see thyroid surgeon on Wed to ‘discuss options’.

In meantime ordered private blood test and published results here on last post. Results were:

TSH 0.685 mlU/L (0.27-4.2)

Free T3 5.82 pmol/L (3.1-6.8)

Free Thyroxine 13.400 pmol/L (12-22)

Thyroglobulin Antibodies 18.400 KIU/L (<115)

Thyroid Peroxidase Antibodies 57.5 KIU/L (<34)

My ferritin and vitamin d are low end and I received great advice on trying to rectify this which I’m in the process of undertaking.

My only confusion really is that I have various symptoms of underactive thyroid with a couple of symptoms of overactive thrown in. My elevated antibodies made me think perhaps Hashimotos but my other blood results don’t seem to reflect this as I would assume my TSH would be raised? I did some reading which suggests that lower end/normal TSH and lower end/normal T4 can indicate secondary hypothyroidism due to pituitary gland issues? This has got me worried.

I realise I’m searching for answers no one here can really give me and probably tying myself in knots over things I don’t really understand. I’ve been looking at private adrenal salivary test/hormone profile blood tests on medichecks but I could be barking up completely the wrong tree. Having a bad day and just feeling a little helpless. I know ultimately I need to see what surgeon has to say. I’m looking to see a private endo once I’ve seen thyroid surgeon.

Any insights would be appreciated! Genuinely appreciate advice received so far.

Your Ft4 is on low end of range

Your ultrasound scan shows thyroid has issues

Ask for trial of levothyroxine

Thanks Slow Dragon. So you think these issues could warrant at least asking for meds — that gives me some hope. I’m clinging to the idea of getting a diagnosis or treatment. For some reason they cancelled my NHS endo apt in place of seeing thyroid surgery (not sure why and I didn’t have energy to question it with GP - it appears to be a diagnostic clinic) but my intention is to go privately to an endo depending on outcome on Wed. Thanks again.

Hi my TSH levels were within the UK acceptable normal limits I.e TSH 3.2 but my antibodies were extremely high so I was diagnosed with Hashimoto by a private specialist. My Gp genuinely didn’t understand the concept that you could have Hashimoto with normal TSH. They are not very clued up on thyroid disorders.

Thanks Snowow! My antibodies aren’t particularly high but I don’t know if that indicates much - maybe they go up over time? It’s all a bit of a minefield! Seems that blood results aren’t always the best indicator. Did you get treatment with ‘normal’ TSH?
