evening all,

I hope you could help with my results I had done by thriva. Doctor claims they are normal but I still feel rubbish.

B12 active 38 pmol range 37.5-188

Ferritin 101ug/l range 44-150

Folate 12 nmol range 8.83-60.8

FT3 5.1pmol range 3.1-6.8

TSH 3.57 miu/l range 0.27-4.3

TgAb 31 ku/l range 0-115

TPO antibodies no results not enough blood πŸ™„

T4 103 nmol range 66-181

FT4 17.1 pmol range 12-22

VIT D 64 nmol range 50-175

I feel like some of these are a bit too low and would also benefit my TSH being lower. I have asked for another tube to do the antibodies.

Thank you so much guys

Your TSH is too high and should be 1 or lower. Are you taking Levothyroxine?

Thank you

B12 is EXTREMELY low

Any active B12 under 70 is considered suspect

Ask GP to do full testing for Pernicious Anaemia

Vitamin D still too low. Aiming for minimum of 80nmol

TSH too high

You need 25mcg dose increase in levothyroxine

B12 active 38 pmol range 37.5-188

Your Active B12 result is dire. A level below 70 suggests testing for B12 deficiency according to Viapath at St Thomas' Hospital:

viapath.co.uk/our-tests/act...

Reference range:>70. *Between 25-70 referred for MMA

Do you have any signs of B12 deficiency – check here:

b12deficiency.info/signs-an...

If you do then list them to discuss with your GP and ask for testing for B12 deficiency and Pernicious Anaemia. Do not take any B12 supplements or folic acid/folate/B Complex supplements before further testing of B12 as this will mask signs of B12 deficiency and skew results.

Folate 12 nmol range 8.83-60.8

Folate and B12 work together. Folate is recommended to be at least half way through range, so with that range you are looking at at least 35+

Once you have been tested for B12 deficiency and started on B12 injections or supplements, then you should also take a good quality, bioavailable B Complex to keep all the B vitamins in balance. Consider Thorne Basic B or Igennus Super B. This must not be started before addressing the B12 with testing and starting injections or supplements as it will mask signs of B12 deficiency and also skew the B12 result.

When taking a B Complex, because it contains Biotin then we need to leave it off for 7 days before any blood tests as Biotin is often used in the testing procedure and will give false results.

Ferritin 101ug/l range 44-150

That's good.

VIT D 64 nmol range 50-175

The Vit D Council recommends a level of 125nmol/L and the Vit D Society recommends a level of 100-150nmol/L.

To reach the recommended level from your current level, based on the Vit D Council's suggestions you could supplement with 3,000-4,000 D3 daily

Retest after 3 months.

Once you've reached the recommended level then you'll need a maintenance dose to keep it there, which may be 2000iu daily, maybe more or less, maybe less in summer than winter, it's trial and error so it's recommended to retest once or twice a year to keep within the recommended range. You can do this with a private fingerprick blood spot test with an NHS lab which offers this test to the general public:

vitamindtest.org.uk/

Doctors don't know, because they're not taught much about nutrients, but there are important cofactors needed when taking D3 as recommended by the Vit D Council.

D3 aids absorption of calcium from food and Vit K2-MK7 directs the calcium to bones and teeth where it is needed and away from arteries and soft tissues where it can be deposited and cause problems such as hardening of the arteries, kidney stones, etc.

D3 and K2 are fat soluble so should be taken with the fattiest meal of the day, D3 four hours away from thyroid meds if taking D3 as tablets/capsules/softgels, no necessity if using an oral spray.

Magnesium helps D3 to work. We need Magnesium so that the body utilises D3, it's required to convert Vit D into it's active form. So it's important we ensure we take magnesium when supplementing with D3.

Magnesium comes in different forms, check to see which would suit you best and as it's calming it's best taken in the evening, four hours away from thyroid meds if taking magnesium as tablets/capsules, no necessity if using topical forms of magnesium.

Don't start all supplements at once. Start with one, give it a week or two and if no adverse reaction then add the next one. Again, wait a week or two and if no adverse reaction add the next one. Continue like this. If you do have any adverse reaction then you will know what caused it.

TSH 3.57 miu/l range 0.27-4.3

FT4 17.1 pmol range 12-22

FT3 5.1pmol range 3.1-6.8

These results show that you are undermedicated with your Levo. The aim of a treated Hypo patient on Levo only is for TSH to be 1 or below with FT4 and FT3 in the upper part of their reference ranges if that is where you feel well. Your FT4 is 51% through range and your FT3 is 54.05% through range so they are pretty well balanced but you need to see what they are like when your TSH is around 1.

Your priorities are:

Addressing the B12.

Improving Vit D and Folate levels.

Increasing Levo to achieve TSH of 1.

Did you check the signs/symptoms of B12 deficiency? Do you have any? If so did you list them to discuss with your GP? They are supposed to take signs/symptoms into account rather than numbers.

For specific advice on how to proceed with your B12 problem, I suggest you pop over to the Pernicious Anaemia forum, list any symptoms that you are experiencing, your B12, folate, MCV and MCH results (with ranges) and see what they recommend.
