Dear all, got my results that seem to be in range:

CRP 0.865mg/L < 3 R

FERRITIN 75.1 ug/L 30 - 332 R

B12 142 pmol/L > 3.75

Vit D 127 nmol/L 50 – 250

TSH TSH 2.23 mIU/L 0.27 - 4.2 R

Free T3 4.3 pmol/L 3.1 - 6.8 R

Free Thyroxine 17.8 pmol/L 12 - 22 R

THYROGLOBULIN ANTIBODIES 15.4 kIU/L 0 - 115 R

THYROID PEROXIDASE ANTIBODIES 11.3 kIU/L 0 - 34 R

I will now ask my GP for Vencamil instead of Teva and will post if I feel any better. Thank you all for you comments.

how long have you been on 75mcg

Free T4 (fT4) 17.8 pmol/L (12 - 22) 58.0%

Free T3 (fT3) 4.3 pmol/L (3.1 - 6.8) 32.4%

Results suggest you have room for further increase …..perhaps initially to 87.5mcg daily (75mcg and 100mcg alternate days)

But if you feel ok …..retest 6-8 weeks after being on Vencamil first

Thanks. I don't have record at what point I was advised 75 mcg having had 50 mcg before, 20 years altogether. I don't feel ok, slow, latargic as always. So you think either increase or 100 every other day?

Discuss with GP or endo to increase to 87.5mcg daily or equivalent

Retest in 8-10 weeks

Guidelines of dose Levo by weight

approx how much do you weigh in kilo

Even if we frequently start on only 50mcg, most people need to increase levothyroxine dose slowly upwards in 25mcg steps (retesting 6-8 weeks after each increase) until eventually on, or somewhere near full replacement dose (typically 1.6mcg levothyroxine per kilo of your weight per day)

cks.nice.org.uk/topics/hypo...

bnf.nice.org.uk/drugs/levot...

nhs.uk/medicines/levothyrox...

Adults usually start with a dose between 50 micrograms and 100 micrograms taken once a day. This may be increased gradually over a few weeks to between 100 micrograms and 200 micrograms taken once a day.

Some people need a bit less than guidelines, some a bit more

TSH should be under 2 as an absolute maximum when on levothyroxine

Graph showing median TSH in healthy population is 1-1.5

Comprehensive list of references for needing LOW TSH on levothyroxine

healthunlocked.com/thyroidu....

onlinelibrary.wiley.com/doi...

If symptoms of hypothyroidism persist despite normalisation of TSH, the dose of levothyroxine can be titrated further to place the TSH in the lower part of the reference range or even slightly below (i.e., TSH: 0.1–2.0 mU/L), but avoiding TSH < 0.1 mU/L. Use of alternate day dosing of different levothyroxine strengths may be needed to achieve this (e.g., 100 mcg for 4 days; 125 mcg for 3 days weekly).

sciencedirect.com/science/a...

The optimal daily dose in overt hypothyroidism is 1·5–1·8 μg per kg of bodyweight, rounded to the nearest 25 μg.

Many thanks. I am 70 kg. My GP refused to prescribe me Vencamil tonight saying it's too expensive and I would need a specialist to confirm that I can't tolerate Teva.

Vencamil tonight saying it's too expensive and I would need a specialist to confirm that I can't tolerate Teva.

That’s absolute rubbish

GP obviously clueless

It’s the same price

Try ordering from specialist thyroid pharmacy eg Springfield pharmacy

My pharmacy confirmed tonight they would try to order from their supplier and they didn't insist on Vencamil brand on prescription, only levothyroxine. I will try to reason with senior GP! I spoke to Mr Springfield today and he does insist on Vencamil brand specifically prescribed.

You can find the tariff price in the link below; this is the price the NHS currently pay for any generically prescribed levothyroxine 75mcg. The price is in pence, so they currently pay £2.63 for 28 tablets of generically prescribed product.

Part VIIIA Dec 24.xls.xlsx

If they prescribe Vencamil 75mcg tablets they actually pay £2.53 for 28 tablets because the list price of the brand (Vencamil) is 10p cheaper than the tariff.

B12 142 pmol/L > 3.75

Think this range applies to folate

Was folate tested

I had a message that they were unable to process your folate biomarker and refunded me £ 8. I am unable to show you print screen about B12 which shows 142 in range of 37.5-187.5. Should I pass them this remark?

B12 142 pmol/L > 3.75

Can you check the reference range on this result. I'm assuming it is an Active B12 test because of the pmol/L units of measurement and the reference range is usually something like 37.5 - 150

Thanks for spotting; it's 142 in range from 37.5 - 187.5
