I am hypothyroid and I have been on 125mg for a few months now after being lowered from 150mg, which i was on for years. I've been feeling a bit rubbish overall but it's hard to know what's caused it as work has been massively affecting my mood recently.

I had an advance thyroid profile done by Thriva and got the results back today.

All my results have come back normal except from my vitamin D which is low. Some ranges also have an optimal part so I'm thinking I should try to move into this range.

B12 Active 67 pmol/L [Normal: 37.5 - 188]

Ferritin 25 ug/L [Normal: 13 - 150] [Optimal: 44 - 150]

FT3 6.3 pmol/L [Normal: 3.1- 6.8] [Optimal: 3.1 - 5]

TSH 1.23 mIU/L [Normal: 0.27 - 4.3 with Optimal: 1 - 2.5]

TgAB 25.5 kU/L [Normal: 0 - 115]

TPOAb 34.9 kIU/L [Normal: 0 - 34]

T4 187 nmol/L [Normal: 66 - 181]

FT4 19.1 pmol/L [Normal: 12 - 22] [Optimal: 12 - 17]

Vit D 29 nmol/L [Normal: 50 - 175 with Optimal 75 - 100]

They've recommended as part of a health plan:

Eating Vitamin D rich foods and taking a supplement.

Taking an iron supplement.

Trying a gluten free diet for a few weeks (Not sure the relevance this has to my thyroid but it says based on my thyroid advanced profile)

Any tips on what I should do next?

You'll need to do some editing of the ranges for your results because there are several obvious mistakes.

I've never seen a "normal" range for ferritin of 13 - 44. And you mention that optimal for ferritin is above the normal range, which is never going to be true.

For FT3 the optimal range is below the normal range, which can never happen.

For FT4 optimal is below the normal range which can never happen.

Whoever decided on those 'optimal' ranges knows nothing about optimal ranges. I would ignore them, if I were you. Optimal, as far as thyroid hormones is concerned, is more about how you feel. And, I'm pretty certain that no-one is going to feel good with an FT3 of 3.1 in a range of 3.1 to 6.8 - and probably wouldn't feel to good at 5, either.

I woudl disagree with a lot of those "optimal ranges" - where did you find them? Optimal FT3 is very individual, but for most people would be more like 5-6 with that range. Same with FT4, but would start at about 50% of range (17), not end there. Personal optimal ranges for thyroid hormones are very, very narrow. Optimal range for ferritin should start at 70 and vitamin D more like 90-125 and active B12 starts around 75

These are the ranges that thriva provided with my results. Thanks for your input on the ranges. I’ll take this into account

How do you feel? Did you take these tests because you had symptoms or just to check?

Your vitamin d is very low. When mine was only a little lower than that I was prescribed 20,000 ui twice per week for six weeks by my doctor. That got my vit d from around 23nmol to around 80. I stupidly didn’t keep it up and went back down. Seaside Susie has some very good info on vitamin d. If she doesn’t come along search her posts on what to do and some of the important Co factors you need to take with vitamin d.

I’ve seen from your previous post that you’re r hypothyroid and on 150mcg levo. It might be worth adding that to your post above so people can help better. Lots of people here seem to need to have a tsh of under 1 to feel better so you might need to increase your dose. But if you add a bit more info to your post someone more experienced than me will help you :)

I've added it now. Thanks for the advice. Yeah i feel like if i remember correctly my doctor has previously said that they want the TSH to be <1 for hypothyroid people.

Wow that seems like a high dose of vitamin D, when you compare to the suggestion that I take 1000ui a day as a supplement. I might have to invest in some higher dose supplements. And thanks for that, i'll have a gander

No worries. And sorry should have been clearer - the initial high dose is a loading dose to get you where you need to be. Then you move to lower dose to keep it topped up. For me my day to day dose would need to be higher than 1,000/ day more like 3,000 but everyone is different :) seaside susie is defo your lady for all things vit d though, the woman is vitamin d encyclopaedia!

I found from my own personal experiences with my thyroid journey that making large changes up or down with my thyroid med dosing does not work well for me . In your case going from 150mcg T4 to 125mcg T4 may have been a big dose change. They missed an in-between dose 137mcg dose . Small changes are more preferred way to go .
