Hi, i am not sure what to do, could someone please help me. My last blood results were jan. 19 :

TSH 0.62 RANGE 0.27 -4.2

FT4 8.3. 12.0 -22.0

FT3. 4.1. 3.1 -6.8

FERRITIN 163. 15-150

Vit D 75. 30-125

New blood results are.

TSH 0.03. RANGE 0.27-4.20

FT4. 11.5. 12.0-22.0

FT4. 5.1. 3.1-6.8

FERRITIN 203. . 13-150.

VIT D. 54. 30-125

I was taking 20 -T3 and 50 Thyroxine. But Doctor up it to T3. 20 and 75 Thyroxine last January . Should i reduce the Thyroxine. I am feeling ok.

Why would you reduce the thyroxine? If you are feeling ok then those results are fine. If you are concerned about the low tsh, don't be. Once we are on thyroid medication the tsh is of little importance - what is important is ft3 and how we feel.

I would be more concerned about the rising ferritin and falling vit D. Can you get a full iron panel done just in case it is a problem?

Thank you. But why should i be concerned. I am due to see my doctor tomorrow and i will ask him .

Because high iron can be a lot more dangerous than low iron and might show haemocromatosis or liver disease - could just be a lot of inflammation. Falling vit D after the summer when it should have been increasing means that you will be very deficient by the spring

Your iron overload is minor at the moment, but it should not be allowed to get really high before you try and find out why it is happening.

Ferritin is frequently slightly raised due to inflammation

Inflammation is common if cause of your hypothyroidism is due to autoimmune thyroid disease also called Hashimoto's

You also need B12 and folate tested

What is the units on vitamin D - nmol or ng/ml

Recommended on here that all thyroid blood tests should ideally be done as early as possible in morning and before eating or drinking anything other than water . This gives highest TSH, lowest FT4 and most consistent results. (Patient to patient tip, best not mentioned to GP or phlebotomist)

Last Levothyroxine dose should be 24 hours prior to test, (taking delayed dose immediately after blood draw).

If/when also on T3, make sure to take last 5mcg dose 8-12 hours prior to test, even if this means adjusting time or splitting of dose day before test

Is this how you did the test?

Your FT4 is very low

You may actually need/benefit from small dose increase in Levothyroxine

my Vit B. Is 329 range 197-717. My Folate is 8.1 dont give range except this : less 3ug/l suggested deficiency.

Vit.D is omnol/L.

I did everything you said, but i didn't take 5 mcg T3 ,12 hour before.

So if you took last T3 about 24 hours before blood test the result for FT3 is probably a slightly false low

B12 and folate on the low side. Supplementing a good quality daily vitamin B complex, one with folate in not folic acid may be beneficial.

B vitamins best taken in the morning after breakfast

Igennus Super B complex are nice small tablets. Often only need one tablet per day, not two. Certainly only start with one tablet per day after breakfast. Retesting levels in 6-8 weeks

Or Thorne Basic B or jarrow B-right are other options that contain folate, but both are large capsules

If you are taking vitamin B complex, or any supplements containing biotin, remember to stop these 7 days before any blood tests, as biotin can falsely affect test results

GP will only prescribe to bring vitamin D up to 50nmol. Previously minimum was 75nmol

Aiming to improve by self supplementing to at least 80nmol and around 100nmol may be better .

Once you Improve level, very likely you will need on going maintenance dose to keep it there.

Retesting twice yearly via vitamindtest.org.uk

Vitamin D mouth spray by Better You is good as avoids poor gut function.

It's trial and error what dose each person needs.

Do you have Hashimoto's?

Frequently with Hashimoto's we need higher dose than average

Thank you so much SlowDragon ,that is a great help. I am taking multivitamins . I hadn't taken Vit b or Vit d as they were always high vit b normal over and yes i do have Hashimoto's . Thank you again for recommending me supplements to take and. good ones, as i never know which ones to buy, as there are so many.🌝

Most multivitamins contain too little on anything if you are actually deficient. Plus most contain iron which will prevent absorbing most of the other.

Most contain iodine which isn't recommended for anyone with Hashimoto's

They tend to have cheap and least absorbable forms of vitamins too

We always recommend only supplementing specific vitamins not a multivitamin

Multivitamin could perhaps be contributing to your high ferritin. You don't want to be supplementing iron without full iron panel, especially with high ferritin. You can have high ferritin and low iron. Ferritin may just be high due to inflammation of Hashimoto's.

Selenium supplements can help improve conversion of FT4 to FT3

Your vitamin D has dropped despite it being end of summer when it should be at its highest ready for winter

Thanks again. I will stop taking the multivitamins and i will also get some selenium as well as the others. I am a little concerned about the vit.d , i am normal at the top (85)end with out taken any supplement .and i have over once or twice. So this is new for me to bbe so low.

Calcium levels should rise naturally as vitamin D improves with vitamin D supplements

Suggest you read up about magnesium and vitamin K2 Mk7 when taking vitamin D

Once vitamin D is improved you could ask GP to test calcium levels

If taking any magnesium (or calcium) supplements these should be at least four hours away from taking Levothyroxine
