Any advice on last blood test below?

25/10/2018

TSH 0.02 (0.27 – 4.20)

T4 total 71.0 (66 – 181)

Free T4 14.20 (12.0 – 22.0

Free T3 4.97 (3.1 – 6.8)

Before seeing Endo – 100mcg of T4 & self-medicating 25mcg T3

22/01/2019

TSH 0.01 (0.27 – 4.20)

T4 total 76.5 (66 – 181)

Free T4 14.50 (12.0 – 22.0)

Free T3 4.97 (3.1 – 6.8)

T3 removed at request of Endo at beg Feb (due back to Endo end July) – 100mcg of T4.

since removing T3 I have put on a stone in weight – ok some of it my fault over eating I get so hungry, can be as soon as an hour after breakfast (stomach pain and grumbling).

04/06/2019

TSH 1.36 (0.27 – 4.2)

Free T4 15.1 (12.0 – 22.0)

Free T3 4.0 (3.1 – 6.8)

I am going back to GP next week to see if he will increase my T4 but not sure if based on these results he will? What should I say? I will be having another blood test before seeing the endo so in about 6 weeks time?

I am taking supplements B12, Folate, D3, Omega 3.

You have Hashimoto's and previous posts shows low vitamin levels

healthunlocked.com/thyroidu...

What supplements have you been taking to improve these

You need vitamin levels retested asap

As you have Hashimoto's...Are you on strictly gluten free diet?

If not ....While still eating high gluten diet ask GP for coeliac blood test first or buy test online for under £20, just to rule it out first

Assuming test is negative you can immediately go on strictly gluten free diet

(If test is positive you will need to remain on high gluten diet until endoscopy, maximum 6 weeks wait officially)

Trying gluten free diet for 3-6 months. If no noticeable improvement then reintroduce gluten and see if symptoms get worse

Other gut issues due to being hypothyroid

healthunlocked.com/thyroidu...

But don't be surprised that GP or endo never mention gut, gluten or low vitamins. Hashimoto's is very poorly understood

suggest you see GP and insist on vitamin testing while requesting 25mcg dose increase in Levothyroxine

Point out your low FT4 and FT3 dropping at each test

New NHS England Liothyronine guidelines November 2018 clearly state on pages 8 & 12 that TSH should be between 0.4-1.5 when treated with just Levothyroxine

Note that it says test should be in morning BEFORE taking Levo thyroxine

Also to test vitamin D, folate, B12 and ferritin

Many thanks for the reply

I had full bloods done in Feb by Endo

Cortisol level 307 (morning cortisol levels not elevated) no ref range ? Still means nothing to me....

Ferritin 67 range 13 - 150

Folate 6.1 range 2.4 - 20.0

B12 633 range 197 - 771

Coeliac test

IMA 1.20 range 0.7 - 4.0

TTG abs (IgA) 0.3 range 0-10 ( not elevated)

You need vitamin D tested

vitamindtest.org.uk

folate on the low side. Supplementing a good quality daily vitamin B complex, one with folate in not folic acid may be beneficial.

chriskresser.com/folate-vs-...

B vitamins best taken in the morning after breakfast

Recommended brands on here are Igennus Super B complex. (Often only need one tablet per day, not two. Certainly only start with one tablet per day after breakfast. Retesting levels in 6-8 weeks ).

Or Jarrow B-right is popular choice, but is large capsule

If you are taking vitamin B complex, or any supplements containing biotin, remember to stop these 7 days before any blood tests, as biotin can falsely affect test results

endo.confex.com/endo/2016en...

endocrinenews.endocrine.org...

Ferritin is ok but doesn't want to drop any more

Eating liver or liver pate once a week, plus other iron rich foods like black pudding, prawns, spinach, pumpkin seeds and dark chocolate, plus daily vitamin C can help improve iron absorption

so coeliac test is negative

Trying strictly gluten free diet is the only way to know if you are gluten intolerant rather than coeliac

You still need to discuss dose increase in Levothyroxine

Levothyroxine recommended doseage is minimum of 1.6mcg per kg

You might be able to use this to argue for dose increase depending on your weight

Such good advice from here . Just add about gluten. I gave up nearly 2 years ago and lost the awful hunger pangs /pains I would suffer within an hour or so after eating a meal .

I could never wait if my food was a bit late . However that symptom settled and I have ‘normal ‘ meal times now .

Hope that helps you also .
