Your TPO result is within range, so not showing positive for Hashimoto's. You do however, have a very low within range FT3 compared with what might be expected with the TSH and FT4 levels, when supplementing with thyroxine; and although better without it previously, even at 5.0 wasn't perfect given your FT4 was 22. When supplementing with too much T4 or supplementing with T4 unnecessarily, the ratio of RT3 to T3 can increase, which might have happened with you. It might be the case that your thyroid is functioning well but that your T4 to T3 conversion isn't optimal for you (a level of 5.0 might suit someone else but perhaps not you), in which case supplementing with a small dose of T3 might help; but I would optimise my other nutrients first, where they are not testing optimal - both selenium and zinc for instance, are necessary for conversion. Low levels of the enzyme intestinal sulfatase can impact activation of the T3 you do have, so good gut health is important; as is liver functioning for conversion. And of course, high cortisol levels can affect conversion too.
T3 is the active thyroid hormone which controls the metabolism and every cell in the body.
Your free T3 in August was 5.0 which is above the midpoint of the normal range, which is good so you are not hypothyroid.
Your symptoms, which continued with 75 mcg levaxin, could however indicate that you have a thyroid problem. One possibility is Impaired Sensitivity to Thyroid Hormone, also known as thyroid hormone resistance. With it you need very high T3 levels to overcome the resistance.
