Advice on Blood Test Results

I was first diagnosed Hypo with Adrenal issues by Dr Peatfield back in 2005 and currently take up to 75mcgs daily of Eltroxin, depending on how I feel.

I'd be very much obliged for any advice/comment on my latest blood (taken after fasting overnight and no meds for 48hrs) test results as follows:

TSH 0.74 (0.35-4.95)

FT4 12.8 (9.0-19.1)

FT3 3.9 (2.9-4-9)

Thyroid Antibodies (HbA1c IFCC) 42 (20-42)

Anti Thyroid Peroxidase (Abs) <0.5 (0 -5.61)

Vit D 54 (0-75)

Folate 6.0 (3.1-19.9)

Ferritin 131 (17-320)

B12 416 (120-650)

First of all, you left off your Levo for too long. It should be 24 hours for Levo (12 hours for T3 and NDT). So you have a false low FT4 and wont know what your normal circulating FT4 is. I have read here that if you add 20% to the result that gives an approximate idea of your level, so you may possibly be looking at 15.36 for your FT4, but honestly, who knows.

Thyroid Antibodies (HbA1c IFCC) 42 (20-42)

That is not thyroid antibodies, HbA1C is a diabetes test and you are at the very top of the range and this should probably be discussed with your GP.

Thyroid Peroxidase (TPO) antibodies are nice and low and don't indicate any autoimmune thyroid disease, however, you can have negative TPO antibodies but positive Thyroglobulin (TG) antibodies so that might be worth testing.

Vit D 54 (0-75)

Is the unit of measurement NMOL/L or NG/ML? If nmol/L then it's low, the Vit D Council, the Vit D Society and Grassroots Health all recommend a level of 100-150nmol/L so it would be a good idea to supplement with D3 to reach that level, along with D3's important cofactors magnesium and Vit K2-MK7. Once that level is reached, a maintenance dose will be needed and that's found by trial and error, retesting twice a year to keep an eye on levels.

Folate 6.0 (3.1-19.9)

This is low. Recommended is at least half way through it's range so 11.5+

B12 416 (120-650)

Again, what unit of measurement? If pg/ml (ng/L is the same) then according to an extract from the book, "Could it be B12?" by Sally M. Pacholok:

"We believe that the 'normal' serum B12 threshold needs to be raised from 200 pg/ml to at least 450 pg/ml because deficiencies begin to appear in the cerebrospinal fluid below 550".

"For brain and nervous system health and prevention of disease in older adults, serum B12 levels should be maintained near or above 1000 pg/ml."

Ferritin 131 (17-320)

This is high enough for thyroid hormone to work properly but it's recommended in some articles to be half way through it's range.

Most importantly though - how do you feel?

Thanks for your very prompt reply and the valuable insight provided.

As regards your queries:

FT4 - Point taken and I will know for the next time - However and whilst it may not be of any significance, in referring to a previous blood test in Aug 2016, a fasting but still taking Levo reading was 15.6.

TG antibodies - Point taken - As my GP obliged all these tests through the hospital lab, I'll sound her out next time for the TG as well.

Vit D - The unit of measurement is nmol/L

Folate - Again in Aug 2016 this was up at 16.4

Ferritin - Point taken but it was only 79 in Aug 2016 and I felt better then.

B12 - The unit of measurement is ng/L - Point taken and in Aug 2016, it was up at 621 and I felt better then. But I also recall my GP telling me to ease off!

Note: I'm not sure whether this is relevant to any of the above but I've always had a low iron count and all the iron tonics etc never made any difference to the count.

And so on reflection, what I now need to do is to re-introduce supplements for the above.
