Hi would like some help please with these high antibodies

I had a test on the 17thOct which I posted, the thyroglobulin was then 753 I had both theses test done with mediceck

on the 17th Feb 18 the result was 1.258 I had the first test done as My GP would not do a test until the year was up so had to wait until Dec, even though I had be very unwell and they had been sent for throat cancer test and bladder cancer test which I new was not the problem

When they tested me in Dec 17 they said I was over medicated and reduced me by 25 mg, when I showed her the medicheck result from Oct she said that just confirmed I must be reduced, so I am now taking 100mg

I am going to write out my test result for this Feb as I post my October results so hope they are still available but will try to put the on as well

I have been feeling real awful all over swollen face trouble walking sleeping muscle and joint so so cold I would be so grateful if someone could give me some advice

I know this might most likely be that I have Hashimoto does it really get this bad?? am trying hard to keep to gluten free diet it's easy at home, I do not use gluten free things as they are all so full of sugar and salt.

I have had thyroid problem for many year ,I had a mayor RTA in my twenty first year lost my hair about 18 months after that wore a wig for many year was very unwell with both my children and was not diagnosed until after the birth of my second son he is now 36

It has been a very rocky road burst appendix Diverticultis Fibromyalga prolased bladder and bowel I am now 67 yrs and finding it very hard to cope I have no faith In GPs and have never been sent to an endo , but from what I have read on here they sound no better than the GP and before you say change your doctor I have change doctor in one practise and then move to another practice

Which is just as useless the only good thing this one is close

Sorry for going on so I am rather at the end of my rope

So any help would be very much appreciated I have tried so many things thank you for reading this rather long appeal

So in October you had these results (copied from that post) when taking 125mcg

All thyroid tests should be done as early as possible in morning and fasting and don't take Levo in the 24 hours prior to test, delay and take straight after. This gives highest TSH, lowest FT4 and most consistent results. (Patient to patient tip, GP will be unaware)

Is this how you did the test?

Thyroid stimulating hormone 0.081 miu/L 0.27-4.20

when GP saw this I guess they thought it too low and reduced dose by 25mcg (incorrectly)

Free thyroxine 23 pmoi/L 12.00-22.00

Yes it was very slightly high and above range

total thyroxine (T4) 120.0 pm0i/L 59.00-154.0

In range and not too high

Free T3 4.33 pmoi/L 3.10- 6.80

This is too low. Like many with Hashimoto's you are poor converter.

reverse T3 10.84 15.01- 75.00

Fine

Thyroglobulin antibody 753.00 iu/mL 0.00 -115,OO

Thyroid peroxidase antibodies 195 iu/mL 0.00 -34.00

These are both very over range and confirms you have Hashimoto's also called autoimmune thyroid disease - see below

Active B12 127.0 pmol/L 25.10 - 165,00

OK

Folate (serum) 5.24 ug/L 2.91-165.00

On low side, supplementing a good vitamin B complex would improve

25 OH Vitamin D 48 50.00 -200

Much too low and needed improvement to bring up to around 100nmol

How much are you supplementing and have you retested since? If not can get test via

Inflammation marker CRP- High Sensitivity 0.7 mg/I 0.00-5.00

Iron Status Ferritin 226 ug /L 13.00-150

Ferritin above range, can be complication of Hashimoto's and/or inflammation

Since reducing Levo to 100mcg you results have got worse (not surprised)

FT3 now is even lower and antibodies are much higher

B12 also reduced

No vitamin D test result?

Your antibodies are high this is Hashimoto's, (also known by medics here in UK more commonly as autoimmune thyroid disease).

About 90% of all hypothyroidism in Uk is due to Hashimoto's

Hashimoto's affects the gut and leads to low stomach acid and then low vitamin levels

Low vitamin levels can affect Thyroid hormone working

Poor gut function can lead leaky gut (literally holes in gut wall) this can cause food intolerances. Most common by far is gluten

According to Izabella Wentz the Thyroid Pharmacist approx 5% with Hashimoto's are coeliac, but over 80% find gluten free diet helps significantly.

Either due to direct gluten intolerance (no test available) or due to leaky gut and gluten causing molecular mimicry (see Amy Myers link)

But don't be surprised that GP or endo never mention gut, gluten or low vitamins. Hashimoto's gut connection is very poorly understood

Changing to a strictly gluten free diet may help reduce symptoms, help gut heal and slowly lower TPO antibodies

Ideally ask GP for coeliac blood test first

Dr Toft, past president of the British Thyroid Association and leading endocrinologist, states in Pulse Magazine,

"The appropriate dose of levothyroxine is that which restores euthyroidism and serum TSH to the lower part of the reference range - 0.2-0.5mU/l.

In this case, free thyroxine is likely to be in the upper part of its reference range or even slightly elevated – 18-22pmol/l.

Most patients will feel well in that circumstance. But some need a higher dose of levothyroxine to suppress serum TSH and then the serum-free T4 concentration will be elevated at around 24-28pmol/l.

This 'exogenous subclinical hyperthyroidism' is not dangerous as long as serum T3 is unequivocally normal – that is, serum total around T3 1.7nmol/l (reference range 1.0-2.2nmol/l)."

Also request list of recommended thyroid specialists, some are T3 friendly

Professor Toft recent article saying, T3 may be necessary for many, otherwise we need high FT4 and suppressed TSH in order to get FT3 high enough

Suggest you try strictly gluten free diet and perhaps increase in Levo back to 125mcg or to 100/125 alternate days

If FT3 remains low then, like many with Hashimoto's, you may need the addition of small dose of T3. So look at seeing T3 friendly endo (or self medicate)

I have done the things you said re 24hr and early morning collection

I had done cortisol 445 133-537

Said normal

Vit D 78 50-200

I was taking spray vit D as the tablets made my stomach even worse than than I am use to but I stopped when they said normal

I have taken multi vits in the past but did not seem to help so I stopped

So are you on strictly gluten free diet? You don't need to have any gut symptoms to benefit significantly

Yes loads of us prefer vitamin D mouth spray. You really want vitamin D around 100nmol with Hashimoto's

So I should go back on Vit D , yes I am now gluten free have only been for a month also Izabella Wenzhou book which is why I went gluten free , I am a rather a yeasty type so I find all the sugar in the gluten free sets me off and with the Diverticultis I am finding getting enough fibre hard I do used golden crack linseed and grind up pumpkin seed and if my yeast will let me I eat Brazil nuts and walnuts

Would it be good to get B vit in a liquid form

I do not drink alcohol or coffee I have lots of different herb tea so hope that is ok

I have tried various vitamin B complex

I most prefer Igennus Super B complex. Full dose is 2 per day. I did that for few months and now take just one per day

I did 24hrs and early in the morning I did have anew vit D test which said normal I will need to look it up I also had a an adrenal test I will try to and the date taken post them I used a vitD spray but after it cam back ok I stopped
