What has your GP done about these?
They are all dangerously low and need immediate action.
Make an urgent appointment and see different GP if yours has ignored these
You will need loading dose of vitamin D
Full testing for Pernicious Anaemia before starting B12 injections
Folic acid supplements should NOT be started until after first B12 injections
Ferritin- needs full iron panel and highly likely an iron infusion
Testing for coeliac disease would be good idea. Blood test is unreliable, needs to be endoscopy
You have Hashimoto's and have been under medicated, leading to these terrible levels of vitamins

Obviously yours are dire. So bloods show adequate treatment, but you are actually severely hypo.
Poor gut function can lead leaky gut (literally holes in gut wall) this can cause food intolerances. Most common by far is gluten
According to Izabella Wentz the Thyroid Pharmacist approx 5% with Hashimoto's are coeliac, but over 80% find gluten free diet helps significantly. Either due to direct gluten intolerance (no test available) or due to leaky gut and gluten causing molecular mimicry (see Amy Myers link)
But don't be surprised that GP or endo never mention gut, gluten or low vitamins. Hashimoto's is very poorly understood
Changing to a strictly gluten free diet may help reduce symptoms, help gut heal and slowly lower TPO antibodies

As vitamins improve, plus gluten free diet, you will be able to slowly increase dose of Levothyroxine

I think there is a glitch with the forum at the moment, they weren't showing when I posted above. Same thing was happening yesterday with posts not showing unless a reply was made to it, and I'm not getting on-forum notifications via the bell icon either.
What about your vitamin and mineral results, what has your GP said - I can't see an answer to that.
