AlanaV1 I thought nothing could surprise me any more, but your GP appears to have ignored ferritin and folate deficiency yet you're being prescribed 800iu D3 which normally, with your current level, a patient would be told to buy their own!
Ferritin 12 (15 - 150)
Your GP should carry out an iron panel, full blood count and haemoglobin test to see if you have any iron deficiency anaemia. If you have then the treatment is generally one Ferrous Fumarate two or three times a day.
For low ferritin it's once or twice a day. I think with your level you should axtually have an iron infusion which will bring your level up in 24-48 hours, tablets will take months. If you are prescribed tablets then I think you should have the maximum. Take each iron tablet with 1000mg Vit C to laid absorption and help prevent constipation. Take iron four hours away from Levo, and two hours away from any other supplements and medication as it affects their absorption.
Ferritin is recommended to be half way through it's range with a minimum of 70 for thyroid hormone to work.
Eating liver regularly, maximum 200g per week due to it's high Vit A content, and including lots of iron rich foods in your diet, will also help.
Folate 2.6 (4.6 - 18.7)
Vitamin B12 201 (190 - 900)
Do you have any signs of B12 deficiency b12deficiency.info/signs-an... If so then please take these results over to the Pernicious Anaemia Society forum here on Health Unlocked for their expert advice. Also include your ferritin results and any signs of B12 deficiency.
You may need testing for Pernicious Anaemia, but your low B12 needs supplementing as anything under 500 can cause neurological problems. See what the PA forum says then discuss their advice with your GP.

For us Hypos the recommended levels are B12 at the very top of it's range and half way through range for folate.
Vitamin D total 48.8 (25 - 50 vitamin D deficiency)
The recommended level is 100-150nmol/L according to the Vit D Council
800iu D3 isn't going to raise your level. It is hardly a maintenance dose for someone with a reasonable level.
You need something like 4000-5000iu daily for 3 months then retest. Personally, I would buy my own softgels (Doctors Best are very good and reasonably priced bodykind.com/productsearch/... )
Once you've reached the recommended level reduce to a maintenance dose, this could be 2000iu daily but it's trial and error and it's recommended to retest once or twice a year to keep within the recommended range. D3 aids absorption of calcium from food and K2-MK7 directs the calcium to bones and teeth where it is needed and away from arteries and soft tissues where it can be deposited and cause problems.
D3 and K2 are fat soluble so should be taken with the fattiest meal of the day, D3 four hours away from thyroid meds.

Hi I have symptoms like scalloped tongue and pins and needles and tiredness, other symptoms I have lived with all my life. Not sure if these are low B12?
The only way to find out is to ask your GP to do a blood test for B12, ferritin, folate and VitD. Also have a look on the Pernicious Anaemia Society website for a full symptom list and see how many apply to you.
