To state the obvious, your folate is low and just below the reference range and your B12 is a baby step across the bottom of the reference range. I'm assuming that you've had a chat with your GP about these results because you mention being negative for PA. SeasideSusie (SS) has excellent advice on this topic (as so many others) and I'm linking to some of her previous replies on the topic that might be relevant to you.
Ferritin is a step inside the reference range but not at the higher level that is typically recommended here for people to use their thyroid hormones efficiently (both their own hormones and any supplemented ones). However, it's not impressive given that you've been supplementing ferrous fumarate since Feb. so I don't know if humanbean has anything to suggest for you. Again, see SS's suggestions.
Your vitamin D level is categorised as the deficiency zone. You need a more decisive intervention from your GP as 800IU is plainly not doing the job and I don't know why your GP is expecting anything different since you've been taking this since 2013. Have your vitamin D results altered at all? Have you been investigated as to why you're not improving (e.g., calcium levels or parathyroid hormone)? You're over the 30mmol/L level at which NICE recommends loading doses (see below) but as you've not improved for 4yrs plus, I should think your GP might consider an argument for them if there's no reason why you shouldn't be given large doses. Failing that, it might be time to start upping your dose for yourself if you can.
How much ferrous fumarate daily? There doesn't seem to have been much of an increase in 7 months. I think you need to discuss the possibility of an absorption problem with your GP, maybe ask for an iron infusion which will bring your level up in 24-48 hours whereas tablets are going to take forever.
You can also help raise your level by eating liver regularly, maximum 200g per week due to it's high Vit A content, and including lots of iron rich foods in your diet apjcn.nhri.org.tw/server/in...

Vitamin D 37.9 On vitamin D 800iu since 2013
Well, 800iu daily for 4 years hasn't done much to raise your level. Unfortunately the guidelines don't allow your GP to prescribe more than 800iu with that level. You will be better off buying your own and sorting this out yourself. I got my level up from severely deficient at 15 to 202 in 2.5 months by buying my own supplements.
As you have Hashi's you very likely have an absorption problem which SlowDragon has addressed in your previous thread. She always advises to use an oral spray for better absorption for Hashi's patients so you could look at Better You. Buy the 3000iu spray and I suggest you take the equivalent of 9000iu daily for 3-4 weeks then reduce to 3000iu daily. Retest after 3 months. When you've reached the level recommended by the Vit D Council, which is 100-150nmol/L, you need to find your maintenance dose. The spray comes in 1000iu as well and you might need 2000iu daily, it's trial and error, so it's recommended to retest once or twice a year to keep within the recommended range. D3 aids absorption of calcium from food and K2-MK7 directs the calcium to bones and teeth where it is needed and away from arteries and soft tissues where it can be deposited and cause problems.
D3 and K2 are fat soluble so should be taken with the fattiest meal of the day, D3 four hours away from thyroid meds.
Magnesium helps D3 to work and comes in different forms, check to see which would suit you best and as it's calming it's best taken in the evening, four hours away from thyroid meds
