Hello, i got my thyroid results back from Medichecks today. I have symptoms of hypothyroidism. I am on 10mg hydrocortisone in the morning and 50mg dhea for my adrenals.

Tsh: 6.92 (0.27-4.2)

Ft3: 5.7 (3.1-6.8)

Tt4: 101 (59-154)

Ft4: 17.7 (12-22)

Tg antibodies <10 (0-115)

TPo antibodies 9 (0-34)

B12 >1083 (140-724)

Folate 5.64 (2.91-50)

CRP Hs 0.2 (0-5)

Ferritin 109 (30-400)

I missed my dose of hydrocortisone for the blood test which i took at 7am.

Any thoughts on this? Not what i expected with my ft3 and tsh not making any sense. I wonder if my adrenals are putting stress on my thyroid function which is declining, but the cortisol is not allowing the hormone to enter the cells?

B0896,

Are you taking thyroid replacement?

Not on a consistent basis. I took two metavive tablets two nights before the test but none before that.

B0896,

As Metavive don't declare the hormone content it is impossible to say whether it would have raised FT4 or FT3 but if it did 2 days wouldn't be long enough for TSH to respond to higher FT4 and FT3.

Thyroid antibodies are negative for autoimmune thyroiditis (Hashimoto's).

TSH is high which can indicate primary hypothyroidism. FT4 is slightly over mid-range and FT3 may be high because of high TSH flogging conversion.

Weren't you seeing a specialist who diagnosed resistance to thyroid hormone?

B12 and ferritin are good but folate is a bit low so you could supplement folic acid for a while.

I was seeing a private doctor who diagnosed me with thyroid hormone resistance. However, the last few months i cant tolerate thyroid medication without experiencing a rapid heartrate. This made me feel the adrenals were responsible, but my tsh is high so im confused by where this fits in to the picture. I had to stop seeing the doctor as it was becoming too expensive.

B0896,

From what I've read it is the TSH which is resistant to FT4 and FT3 levels so TSH continues pumping out even though FT4 and FT3 are adequate/high. FT3 5.7 is high in range so taking thyroid replacement will increase it and that will make you over medicated and feel very unwell.

Why don't you ask your GP to refer you to NHS endocrinology? RTH is outside of the sphere of most GPs and endos but endos can access tertiary care at Addenbrookes for advice.
