First of all with that B12 you must go and talk to your GP and ask to be tested for Pernicious Anaemia, I imagine you need B12 injections. Folate is under range and it's usually suggested to take methylfolate, normally in a B Complex, when supplementing with B12. See what they say over at the PA forum.
Ferritin needs to be half way through it's range with an absolute minimum of 70 for thyroid hormone to work. Your GP won't help with that level so you could get an iron supplement and take each tablet with 1000mg Vit C to aid absorption and help prevent constipation. Take iron 4 hours a way from Levo and other supplements and meds as it affects their absorption. You could very well get your level up just by eating liver once a week, I've got mine up from a similar level to over half way through range just by eating liver.
Vit D is recommended to be 100-150nmol/L. You can supplement with D3, my preference is for softgels as they usually contain olive oil which helps absorption, and don't have unwanted fillers. I would suggest 5000iu daily and re-test in 4 months. If you've reached the recommended level then reduce to 5000iu alternate days as a maintenance dose. When supplementing with D3 we also need it's important co-factors which are K2-MK7 and magnesium. D3 and K2 are fat soluble so should be taken with the fattiest meal of the day. Take D3 four hours away from Levo. Magnesium is calming and best taken in the evening. Don't start all supplements at the same time. Start with one, give it a week or two to see if there are any reactions, if not add in the second one, give them a week or two and if no reaction add in the next one. That way if there is a problem you will know what caused it.
All said here is true, just a warning, don't start any b12supps now, they skew the testing you need to do to find out if you have PA, or rather, its precondition. So no folate supps yet, and no B12.
