miffed!!
I had a letter last week with the results of tests to see if I have RA or
PsA
. Rheumatologist has said it’s RA even though I have
PsA
symptoms. My hands show extensive grade 2 synovial thickening on both my hands and wrist . Right hand worse and changing shape.

Are PSMA negative mets usually castrate resistant?
I had 2 clear PSMA-PETs (at a
PSA
of 2 & then 4) so Dr. Scholz recommended I get an FDG PET. Low and behold 3 decent sized mets showed up. In my subsequent research it seems like all the published articles describe the PSMA negative mets as being castrate resistant.

Treatment Options PSA Increasing While on Lupron
Should another ADT like Zytiga be added if
PSA
continues increasing by a small amount? Say to 0.40. Or should I wait until
PSA
rises considerably. If
PSA
shows sign of going down, should I continue only with Lupron until
PSA
is undetectable (<0.1)?

His
PSA
dropped to <0.05. This lasted until September 2020 when his
PSA
started to increase 0.46. And continued to rise. Went to the UK for a PSMA scan as none available in ireland. Scan showed 1 pelvic lymph node involved.

That worked well and my
psa
dropped back in the low single digits. Later started rising again. I had been taking Colostrum (mothers milk) for over 40 years for my general health which was very good. I was the healthiest guy for my age I knew. Never got sick.

The ‘shout out’ is for ovarian cancers so I may not have had an immune response but I have heard nothing formally except last weeks scans still showed NED and
PSA
is still undetectable 11 months after finishing all treatment bar ADT and Erleada May be of interest

Orgovyx and stage four
My
PSA
has been at .6 after Cyberknife on my L2. I am trying to get assistance but in the meantime I started taking it every other day and Dr Oscar Goodman said that is ok. I have not taken it in 4-5 days. I have 4 pills left. Am I playing with fire?

Treatment Update
My
PSA
remained nearly constant at 0.30. 10/17/22 0.286 1/20/23 0.321 4/24/23 0.294 If
PSA
has gone even by small amount, my MO would have ordered PSMA CT scan. Is it advisable to have the scan now because
PSA
has not decreased significantly.

Pluvicto works but at what cost
Previously on Firmagon, Xtandi worked for awhile, nothing with Xofigo and a few rounds of Jevtana didn’t help and caused problems, also had localized XRT - but had to have chemo to qualify for Pluvicto which is a great advanced treatment for many of us After multiple delays getting Pluvicto with

Odd ways of discovering you had PCa
PSA
returned as 9, had RALP the next month. Ironically rash disappeared after surgery. 9 months ago, rash came back, my
PSA
hit 0.2 then 0.4. Started Zytiga and Lupron. Rash disappeared again!!! Coincidence?? I cant believe it was.

Fluctuation in free PSA (fPSA)
I am on AS and diligent about the
psa
test. Thankfully
psa
readings have been stable. However, fPSA fluctuates between 21% and 28%. Current one was 26% why the fluctuation in the fPSA level? Any significance to the fluctuation ?

Stinging/burning near tip of penis when urinating. Also, small brown sediment in urine.
Because of rising
PSA
value, will see a new oncologist next week. Radiation therapy 14 years ago,
PSA
rising annually, now at 2.5. Also, there is hip and thigh pain, more so at night. And an odd, mild pain in the groin. And a burning lower back pain, only at night. Any similar experiences?

When, or if at all, to add Nubeqa
RP in 2020 Gleason 7 (4+3)
PSA
14 Been on Orgovyx for 6 months of an 18 month prescribe. Just finished 39 daily IMRT treatments.
PSA
remains <.05 Should I be thinking about adding Nubeqa or not? And if so, when?
