Just completed a whole gland TULSA procedure
This was salvage treatment after failure of SBRT by MRIdian in Mar 2020.
PSA
nadir after SBRT was 2.4 and it rose to 5.3 just 3 years later, when a PSMA scan showed 2 possible tumours within the gland. A subsequent biopsy confirmed local tumour recurrence with Gleason 4+4.

Happy New Year and Update
I will have had 31 months of Lupron with mostly undetectable
PSA
. I will have had 29 months of Abiraterone at that point. Had IMRT radiation 24 months ago Both my MO and RO are interested in whether they have " CURED" me of Oligometastatic stage IV, Gleason 9.

Still Frustrated
As I’ve posted, it began 5 months ago with higher

Magnetic Resonance Spectroscopy
Have contra-indications from multi-parametric MRI (4/2023) and latest serum
PSA
(4.4 ng/ml, 9% free
PSA
). SO recommends retest and another MRI in 6 months. Verifying that prev MRI did not include spectroscopy. Im thinking MRI w spectroscopy is the next step if it wasn't part of previous MRI.

Wondering if anyone can relate to these symptoms

levels have fallen and my progress seems ok. Re. EPI - my haemoglobin and red blood cell count has slowly, slowly dropped over past few years until slightly below lowest ref level but GP said not low enough for concern.

test came in at 16, up from 8.5 in May of 2023. My conclusion is that my cancer is slowly growing. So, I've decided to pursue "IHT," Intermittent Hormone Therapy, six months on, twelve months off, repeat.

HDR boost Brachytherapy + IMRT
I’m currently on Lupron and Zytiga, and
PSA
is dropping like a rock. So the PC seems to be responding nicely to hormone therapy. All RO’s I’ve met with want to wait 2-3 months for the ADT to do its thing and shrink the primary tumor and lymph nodes before they that radiation start treatment.

Post treatment post ADT time to PSMA test
I’m assuming that since my current
PSA
level is so low (.01) a scan at this point would be pointless. At the same time adjuvant ADT should be taken longer if there is some metastasis. I guess the best scenario would have been to fly overseas to get the scan beforehand but I didn’t.

Any Curative Treatments after RT as Sole Primary Treatment?
PSA
has been rising, but not officially nadir plus 2, but the old 3 rises in a row has been reached. If there is nothing curative to be done, do you just wait until nadir plus 2 is reached or something shows up on a scan? is there some way to be proactive? Tall_Allen didn't you have initial RT?

My prostate was removed 2002 at which time my

went to -0 . Five years later it came back and since than it kept on going up very slow till 2020 when I had COVID-19 which almost killed me . As soo as I started feeling better my

as is starting to rise on Arbi/Dexa. I have the CDK12 mutation which is apparently known to be a drug target or to predict sensitivity to Atezolizumab (Tecentriq PD-1 Checkpoint Inhibitor), hence there is open Immunotherapy trial called BoB (NCT03767075).
