A very close friend (82 years old) was admitted to the hospital on New Years eve with severe back and Chess pain. After tests, scans, MRI and blood tests he was diagnosed as follow:

PSA 3882 up from 7.6 the prior year.

stage 4 prostate CANCER (T3/T4) that has metastasized to his spine, lesions on all bones. and one spot on his lungs . Doctors at the hospital recommended the following:

IV Decadron 4mg 4x/day which started on 1/3'

Bicalutamide 50mg once a day for 14 days

Lupron after 2 weeks

No surgeries due to his age. He is high risk for paralysis.

He set up an appointment with an Oncologist for 1/15. Ay recommendation is appreciated

Why can't he have triplet therapy? Too frail?

He will be meeting with his oncologist on 1/15 for the first time. What he is getting now is from the doctors at the hospital. The plan may change once he meets with his MO

If not the triplet due to the chemotherapy part, adding Enzalutamide or Darolutamide might be the way to go. Hopefully your friends MO will discuss all options.

Aren't Enza and/or Daro already one leg of the triplet therapy?

I believe they are

Yes they are but his oncologist may feel that one of these can be added along with the ADT even if not adding the third, chemotherapy, leg. It would be a double therapy approach. This is what I am on.
