Pluvicto failed - what is next?
I just got my most recent
PSA
test after my fourth injection and my
PSA
has now gone from 20 to 30. Also my PSMA scan says that my cancer is no more stable and on the move with more tracers. Still just in the bones. Pluvicto was brutal on my body. Extreme fatigue, dry mouth, terrible.

Neuroendocrine Prostate cancer (NEPC) questions.
Another medical article I read last night said that most of the people they get diagnosed with neural endocrine prostate cancer have a
PSA
below for when diagnosed. Mine is currently below four.

Another notice of passing that prompt me to update my profile.
"Ablin (MD) was the guy who discovered
PSA
. It's actually a pretty interesting video: it claims that life expectancy will not improve and that the treatments harm a significant number of people.

It's Back!
Anyways, 13 years ago I was diagnosed with PCA Gleason 7
PSA
44. With spread outside of prostate. Did 39 treatments off radiation and have been mostly on ADT for the duration except during COVID I took a break of 18 months or so. Went metastatic about 3 1/2 to 4 years ago and started Xtandi.

castration after 7 years of ADT
My
psa
is now slowly rising and ghastly taken 2 years from undetectable to 0.65. I am considering surgical castration now as it would mean no more 3 monthly Zoladex injections and a reduced chemical load on my body. I am fit and well , 83 years of age, and no heart or diabetes problems.

8/4/2023 Update
HOWEVER, 5/23
PSA
went UP to 0.343 and 8/4/23 now 1.46 at my PCP physical. Have MO appt 8/16 which includes another
PSA
check but looks to me like another PET scan and/or Xtandi. I would appreciate any thoughts from all of you to better prepare me for my 8/16 appt.

How can one person be so Lucky?
On the one year anniversary of the original PSMA Pet scan the MO ordered another PSMA Pet scan because he said that in 5% of PC cases the PC could still be growing but not showing up on a
PSA
test.

Rapidly Rising PSA
I was originally deemed to have a non-aggressive cancer (in 14 years my
PSA
has never exceeded 4.1) However, over the last 6 weeks (when I have been on no treatment whatsoever, other than 3 monthly injection) my
psa
doubled in 3 weeks (4 - 11) and doubled again in 3 weeks (11-22).

Age and timing for Triplet therapy?
In December the
PSA
rose to 3.78 and a PSMA-PET scan showed cancer growing in prostate, shrinking in pelvic node, and spread to one abdominal node. I started ADT with Ogovyx 4 weeks ago and feel no side effects so far.

Surgery vs SBRT Cyberknife
PSA
3.7-4.2. Met with Surgeon first, "slow growing, non aggressive" he believes is contained by looking at margins and pathology. Met with Radiology Oncologist, "favorable intermediate risk, non aggressive" he believes it is contained.
