So about 2 years ago, I started getting severe testicular pain and I was diagnosed with epididymitis. My doctor prescribed doxycycline for 10 days and it eventually went away. Over the past year or so, when I wear boxers and they ride up, the boxers cause testicular pain and about 2 months ago, I had a sudden change in my semen volume and appearance. I started getting severe testicular pain and visited the doctor again. The doctor said it was probably caused by trauma from sex but ordered a scrotal ultrasound. During the ultrasound they found a 3mm spermatocle present in the epididymis bilaterally but no vericocle was found. The pain got worse but gradually got better and my semen started to resemble the way it should but I dressed up a few days ago and my boxers caused a wegie and they started causing pain in my testicle again and I’m worried that the pain is coming back. I’m not a doctor but I get the feeling that tight underwear causes the spermatocle to get inflamed.

Hello, it seems that this is a problem about testicular pain. Two years ago, your testicles suddenly ached. The doctor prescribed doxycycline for 10 days. After you ate it, the symptoms disappeared. Did you check it again? This is a problem.
In some patients, although the symptoms disappear, inflammation has not been thoroughly treated, combined with antibiotic resistance, future recurrence is possible.
