hi can you tell me where you got the information ?

i opened the link and then it shut down before i could check it.. bad computer ..

It looks useful but is it the same info that is on a Gov sight or in a clame pack ?.

Thanks very much for this link. It is very timely. I am awaiting my ESA Questionnaire any time soon (received phone call on Monday to say they are sending it), so I am expecting this to give invaluable help in filling it in, have already printed out all 55 pages

It opens as a PDF file, so you need to have Adobe on your computer, otherwise it won't open.
