Contact DWP and ask for your assessment report & ask for a Mandatory Reconsideration. Go through the report with a fine tooth comb and look for anything that doesn't reflect on how your assessment actually went. Look at what they've said for each criteria and why they say you don't get points, be aware they make a heck of a lot of assumptions. If you can get a letter from GP or OT or Physio who can back you up so much the better.
When it comes to scoring points the buzz word is "Reliably" if you can't do something

To an acceptable standard
Repeatedly
In a reasonable time frame. - This means not more than twice as long as the maximum amount of time that a person without your health condition or impairment would normally take to complete that activity.
If you can't do those then that can't be seen as being carried out "reliably".
If possible get help from Citizens Advice or similar organisation. I was lucky, my MPs office has staff who deal with benefits and helped me enormously with my claims.
