ESA new claim

I am being finished from my job soon or can finish myself as I have been off sick for 6 months due to medically not being able to do my job. How do I apply for ESA ? Is there a number to contact them ? TIA I was off work for 1 year sick and then went on ESA and returned to work for 12 months but I can’t remember how I applied for it

It’s online. I didn’t know anything about it till recently and I finished work on Feb. Not heard anything. I’m hoping I’ve got enough stamps as only got half for the last 2 years that they base it on. DWP
