Hi there if you get an appointment to see your doctor and ask if he would support your claim for esa with a sick note. When filling in the form tell them about how it prevents you doing things and how long if that has changed since you've become ill. Really hopes this helps. Also if you look to the right of this page you'll see pinned posts.
Down the list you'll see helpful benefit links. x

ESA comes in two forms, contributory ESA based on your National Insurance record which means you need to have worked in recent years, or Income based ESA which is fully means-tested. Contributory ESA is only mans-tested against pension, annuity and income replacement insurance income.
fibromyalgia Action UK has in the past supplied members of this forum with ESA guides produced by Benefits and Work that take you through the forms step by step. you should try asking them for the guide. Citizens Advice can also help you fill in the forms. Th first claim form is easy but the detailed one is called ESA50 and is used by the Disability Healthcare Professional who will meet you to assess your disability. DWPnthen use their report to decide whether you get ESA and if so which group you are put in, the Support Group or the Work Related Activity Group (WRAG). If it is the WRAG you will be required to participate in activity to prepare you to return to work, and ESA would stop after a maximum of 12 months.
It is not something that I have done myself but I want to sincerely wish you all the best of luck with this. I have pasted you a link to the FMA UK benefits helpline, as this may be useful to you:

All my hopes and dreams for you
