I am agonising over IVF transfer protocols. After a year of batch stimming I am very fortunate to have 3 euploid blastocysts on ice ready to transfer.
I have had five failed transfers (x2 miscarriages, 3 BFNs) and feel like I have tried everything already in terms of protocols (medicated long, medicated short, natural, natural modified + steroids /IVIG / intralipids).
I have had some issues with fluid in the cavity which they are hopefully now treating (I have surgery planned to treat a defective c section niche from first pregnancy), but I also have mild adenomyosis / endometriosis and hard to say which is driving the failures.
Wondering what protocols people have tried for mild endo / adenomo in the UK. Have heard some people are on a longer down reg plus letrozole for this and are skipping the bleed but not sure if that’s available in any UK clinics (mine haven’t mentioned it to me). And does aspirin help with this or is that a different set of issues? Gahh such a minefield.

Adenomyosis sufferer here - longer down regulation (2-3 months), clexane, steroids, progesterone injectables were all added to my last cycle. I’m currently 7 weeks (although viability scan is next week so no idea if it’s heading in right direction). This was 7th transfer.
congrats and fingers crossed everything goes well at the viability scan / it’s 7 times lucky for you. That’s v. helpful. Can I check what meds you were on for down reg? Was it buserelin / suprefract or similar?
It was Arvekap which is Greek but it’s similar to Prostap (one injection monthly). If you’ve got adenomyosis down regulation is a must in my view - depending how bad it is.
Interesting, thanks. I haven’t come across that one. They have said it’s just mild but I am paranoid as I have painful periods and have had 5 failed transfers (plus prior to my last miscarriage they couldn’t visualise the embryo at the first scan because the adenomyomis was obstructing the view on the ultrasound, so wondering if it had flared up). So keen to explore further x
