AMH - Please can someone give me some clarity?

Hello,

Any feedback would be very appreciated.

I did an AMH test last year, it came back at 1,16 NG/ML, which was my baseline scan. This year we did a new AMH test in a different laboratory which came back at 0.58 NG/ML. (Day 2, FSH came back at 8,4 mUI/ML) I became so horrified and anxious that i ran back to my baseline test clinic and did another AMH blood draw within the same cycle, and it came back at 1.02 NG/ML. (Mid-Cycle)

0.58 makes me very nervous. Any idea of what has happened? What should i count on? I always heard that people with an AMG higher than 1, in general should be considered having a good reserve to conceive. 0.58 is almost half of that.

Hi Sancor, I know it's really hard to do but try not to get too fixated on the numbers. I had mine tested a couple of months ago and the clinic queried it (because they thought it was high for my age), got it done in a different lab and it rose by 2 (p/mol) a couple of weeks apart so it's not quite as fixed as the clinics would have us believe. It's also only really useful to see how you might react to stimulation drugs. You could have the highest AMH, a big reserve but they could all be crap quality or you could only have a few left but one could be the one that works. I don't know why it changes and maybe it's different lab techniques but all I did was focus on what I can control which is making sure what eggs are left are in as good shape as possible, feeding them well with healthy fats and proteins and lifestyle changes. You have age on your side and 2 good results so try not to stress too much over the other result.

Hi Sancor. Well the FSH level was done on the correct day of your cycle and is a good reading, saying that your ovaries can be stimulated. The AMH can be done at any stage of your menstrual cycle, and is unfortunately very low, indicating that your egg reseerve is minimal. Your clinic might be willing to give you a go to try and get one or two of your own eggs out to be fertilised, I don't know - one for your specialist to speak to you about. Hopefully there are a few eggs to be used. Good luck! Diane

Hello Diane. Thanks for your reply, I was of the understanding that any AMH over 1 NG/ML which should be around 7 pmol isn’t that bad of a reading and should be able to produce children? I am clinging on to hope that my 1 test is above 1 which seems to be the borderline. Did I read that wrong ? Just did egg collection, got 6 mature eggs, 5 fertilized and 2 good blasts.

Hi. I am so sorry I have been looking at AMH in pool/L and not ng)ml. Normal is over1.0ng/ml so with your normal FSH reading hopefully you will get good results. Have been used to nmol/L Diane

People conceive with AMH lower than that; 0.58 ng/mL is 4.1 pmol/L, which is low but not awful. The 3 different values will in part be due to inherent variability in the assay (measure the same sample 3 times you’ll get 3 different numbers) as well as a bit of natural variation. Your age is important-even though your reserve is low for someone your age, your quality should still be pretty good so you are definitely not out yet! If you’re trying naturally and ovulating each month, AMH doesn’t actually matter that much for now. Xx

I agree with Purpledoggy my AMH has measured at 3.5pmol and 9.3 pmol. I have had two fertility clinics both very willing to take me on and although I have yet to have a live birth I have made blastocyst embryos and become pregnant (sadly I had to say goodbye to my little one). My current clinic, a well respected London clinic, use my follicle count as a major indicator as to my reserve and do not use the AMH or FSH in isolation. Xx

Thank you for your kind message and reassurance. We did get 10 eggs in first round and 6 eggs in second. We will keep trying 🥰
