I had one fitted last year during a diagnostic laparoscopy, struggled with ongoing pain and bleeding and subsequently had an MRI that showed the same as you describe. I’ve just had a second lap to free off the uterus and remove (what has not been confirmed as) endometriosis on that right side. I had the mirena coil removed at the same time. I feel so much better, but I couldn’t say whether that is the coil removal or the surgery or both! I’d say discuss it fully with the surgeon. They (hopefully) would know the scan results and wouldn’t place it if it wasn’t safe. Remember you can always have it removed if it doesn’t help!
I won’t be talking to the surgeon who will do my lap for months. When he gave me my MRI results the other week, he just said give the Mirena coil a go and get it fitted at an STI clinic. They won’t have much of a clue about my endo issues so I’m feeling a bit worried to do it x
Could you ask to have it done at the hospital? Or ask your gp to query it and/ ask for you to have it done at the hospital on your behalf? Gp may be able to allay worries for you ?
family planning clinic’s will be more informed than other physicians around this condition.
Thanks for posting this Christina! I'm sorry I can't help answer your Q, but I have had the same Qs on my mind for a long time and it completely put me off having a mirena fitted.
I'm currently awaiting a lap as it's been strongly suspected I have had endo for 9 years now (have all the classic symptoms plus painful bladder, IBS and fatigue made worse by my cycle). I had an MRI at a private hospital last year that I was really disappointed with as it was super quick (10-15mins) and had no contrast. It did not confirm endo but did show my uterus was so far tilted to the right that it completely "abuts" my right ovary and that I had a number of cysts with "no obvious blood products" on that ovary.
I was told by my Drs that the side tilt is a variation of normal as long as the uterus is also anteverted (tilted forward). An ideal uterus will be anteverted, anteflexed and in a near central position Apparently problems usually only arise if the uterus is also retroverted (tilted backwards), but not in all cases of retroversion. Your MRI report will tell you whether your uterus is also anteverted or retroverted.
I disputed with them as I felt that something must pull your uterus so far out of position, but they maintained it was not a sign of adhesion and was normal. So I wondered if adhesions were visible on your MRI or just the tilt? I also experience left sided constant pain very low in my pelvis and back that gets worse between ovulation and menstruation. I thought this could be the ligaments being stretched by my uterus (again told it wasn't). I wondered if you would mind sharing if you have any similar pain?
I was recommended to have a mirena fitted a few weeks ago, they seem to really push it. It's the 6th or 7th time I've been asked to try it. Like you, my thoughts are surely it will cause more issues if the uterus is dispositioned. I worry it will be painful.

My recent MRI results are as follows- uterus tilted far to the right (I don’t know any more about the positioning), uterus touching the bowel, cysts in the uterus wall, ovarian cyst and subtle nodularity of the peritoneum.
I am with Manchester Royal/St Mary’s endometriosis specialist centre. They said a uterus tilting to the right is strongly suggestive of adhesions. They have listed me for excision surgery based on my MRI.
I had a laparoscopy at another hospital last year and they diagnosed peritoneal endometriosis. My uterus was not retroverted on the pre op scan they did or tilting to the right. My new Gynaecologist feels my endo has progressed since then.
An MRI could show absolutely nothing and you could still have endometriosis, so anything out of the ordinary on it is suspicious. If you’re with a general gynaecologist, try and see an endo specialist. If you already have one, you’re entitled to a second opinion.
My symptoms are fatigue, bowel symptoms, bladder issues, general illness and sciatic pain on the right hand side.
I also had this! My uterus is tilted to the right, my consultant thought I had a unicornate uterus, but MRI showed that it was being pushed to the side by cysts. When I asked if this is a symptom or sign of endo they said no! But I get the sciatic pain everyday and the pelvic pain, bleeding and many other symptoms x
Interesting. I suppose they can all interpret an MRI differently. Your symptoms could be adenomyosis rather than endo or vice versa. I don’t know which of mine is causing which. I’m having the Mirena to help the adenomyosis, so that might give me a clue. X
nobody has mentioned the possibility of adenomyosis to me, i need to do a bit of reading up on it because it confuses me! Yes I agree, MRI can be interpreted differently, I can’t see how your womb being pushed to one side by cysts can not be a symptom of something but this is what they’ve said to me 😂. I hope the coil helps you and eases things down x
The reason I’ve been told I have adenomyosis is based on cysts in the wall of the uterus seen on an MRI. They believe titling is an endometriosis adhesion.
The findings on your MRI are definitely not nothing. They may be interpreted as different conditions but interpreting them as nothing is completely wrong. If you can get hold of you MRI results and access a second opinion, I would.
Even if your MRI showed nothing, the symptoms you’re having are enough of an indicator that something is clearly wrong. X
