White head spots are excess sebum in the skin - could be a sign you are not drinking enough water to sluice out toxins in the skin and keep it suitably moistureised from the inside, but could also be from the hormones in the livial. Have you tried stopping the HRT for a couple of weeks to see if that helps.
Stopping livial is not dangerous, you can take zoladex without HRT up to 6 months anyway, and the HRT is only added to help out with the side effects of the zoladex. Stopping the livial should cause you to have more hot flushes which means sweating out of pores and cleaning them out . Remember to keep washing with antibacterial soap and exfoliating to get rid of the old dry dead skin cells too that coud be trapping the microbes in the pores causing the whiteheads.

At the age of 48 it sounds as if it could be hormonal related to me as. Would you consider yourself to have reached & established your menopausal stage in life?
Before I knew I might of had endometriosis I used to get bad & acne like spots on my face chest & back but throughout the years I've found using an antibacterial soap to bathe with every morning has helped to clear & kill any dead skin cells which may have been hidden in or on my skin.
It may also be very beneficial for you to drink plenty of water based fluids too.

I got the exact same - loads of white spots when I was on zoladex (with no hrt) so I would guess its the zoladex. I used fucibet antibiotic prescription cream on them and they cleared up. Was horrible, rows upon rows of them so I feel for you x
5%benzole peroxide used morning and night will clear up acne. 2.5% is even better as it can be drying but its impossible to get hold of in the UK at present.
Sudocreme is the best treatment I've found for acne. It helps dry up and reduce sebum, while assisting with healing. It can get rid of a spot within a 24-48 hour period. Best to do a thick layer face mask overnight and exfoliate & moisturise with e45 in the morning.
Be warned it will leave white marks on sheets. You can also put a very thin layer on under a powder foundation to help healing during the day.
