I am finding that the effect of glucose or insulin has quite an effect on mobility. Before, it seemed that protein alone would interfere with L-Dopa. Now it's not only that, but post-prandial somnolence that can lay me down for hours. Come the next day, I can pretty much lead a "normal" day (with L-dopa etc) with complete function until... I eat something substantial. So... I fast most of the day. I am following my best attempt at a ketogenic diet for the last three months. I have not been diagnosed a diabetic, and my blood glucose is rarely over 100. My ketones land in the ,5 to 1 MM range, Perhaps it hypoglycemia? hyperglycemia? ... Any thoughts welcome! Thanks in advance.

This sounds very familiar to me. I am intermittent fasting and basically I eat sugar free, gluten free and dairy free somewhat on line with the Wahl’s diet. I have oatmeal at 11 am and dinner at 6. My dinners are a sheet pan of as many vegetables as I can find with multiple of colours and then meat (chicken, sausage, pork tenderloin, steak etc). The timing is crucial because it is away from my medication. Since I started this I feel quite a bit better. Cheers, I hope you find something that works for you.
For me gastroparesis is a constant sign of PD. When I eat I can burp up food hours later. So I assume I'm not absorbing medication completely. Also high-protein meals first digest into amino acids that are used as needed. Any extra food is repackaged back into sugar. I was on an Adkins website and members complained about meat causing blood sugar to be elevated. I have urinalysis strips and can test urine for sugar and ketones. I generally have low to moderate ketones in urine. I don't eat til noon and have a 2nd meal around 6 or 7 PM. That said, I don't seem to be losing weight.
I don't take dopamine replacement, so I can't speak to that, but I do intermittent fast and eat a strict keto diet, and I THINK it has helped keep my PD from progressing. I eat meat, but not a lot, and I eat dairy...but only very high fat, like full double cream and butter, so I suppose I'm not really getting any milk protein or lactose.
I also eat gluten, but I don't happen to react to it, and it's very helpful for baked goods on a keto diet (it's restrictive enough as it is! :o)
I also eat a lot of vegetables, as long as they are above ground (root veggies are very high carb) and I generally drench them in either olive oil or butter.
I don't have diabetes either...though I was heading that way before I was diagnosed (2015). IMHO sugar is one of the worst things a PWP can do to themselves. But, like I said, IMO.
