Do you tell them during the appointment that you have met with and connect with another doctor better or just not reschedule anymore? Or tell the MA’s to tell him you’re not coming back after the appointment?

I don't know what country you are in but in the UK you just go to your chosen practice and ask if they are accepting new patients. Then you just have to provide proof of identity and they will do the rest. x
How you do it is up to you. Whatever feels comfortable.
You are probably not the first patient to change practices with this MD. I'm sure they are used to it.
Only change Doctors as a last resort. They have knowledge of your conditions .
When you change Doctors they will generally ask for a medical get to know you before you attend as a Patient. They will ask your old surgery for your medical records.
Make a list of your problems, Take it with you to the getting to know you appointment, this will help the new Doctor to understand your health conditions and concerns.
Remember depending on how long you have been with the old practice they will have a more inclusive understanding of your Health
Well I have been with my practice 23 years Bob and rarely get to see the same doctor so none of them know me from Adam! Your experience is very different to mine. x
Yes in my practice I can see any GP, although now since early this year I see a selected GP who specializes in PsA. She also did good by referring me to the Aged Persons Clinic.
At this time She is a good choice, although She only attends two/three days a week and I have to make an appointment on a Tuesday.
My health is fast becoming more complex, with my blood pressure loss and Congenital Short Term memory. They aer beginning to reduce medication doses so one Doctor looking after my health is preferred. When probable new medications will be introduced
