Hi All , I wonder if you have any any insight to this ? my husband started chemo (Docetaxel on the 9/10/23 and is coming up to his 3rd treatment due on 20/11/23 he has tolerated the chemo well and is still working which is great . he was dehydrated and his haemoglobin was quite low so they gave him a blood transfusion. now before he started his chemo the oncologist said that because his PSA was rising over the summer after radiation . finished radiation in may and Psa started to rise it was 9 then 6 weeks later 13.5 then 6 weeks later 38 he had a scan and showed some spots had resolved but other small spots had come up in other places .so she said he was castrate resistant and he should stop taking Erleada and go for chemo . she took his PSA on 5 October and it was 87 so another big jump . now my question is he is castrate resistant or is the big jump something to do with the fact that he stopped Erleada ? I know it was going up anyway but that seems a big jump to me . she said she wanted to do a scan after 3 treatments to see if chemo is working .now she is saying lets see what the spa test reads first and make a decision then . I also asked her to add maybe Zytiga into the mix he is also having zoladex and Xgeva but she says she is waiting for genomic testing results before she adds anything else . is this normal practice or should I be fighting for something else to be added . any info will be much appreciated . thanks carol

Maybe talk about adding enzalutamide with the chemo. It usually doesn't work long after apalutamide fails, but docetaxel may prolong its effectiveness, as it dod in the PRESIDE trial:
