72 y/o. Diagnosed with prostate cancer Dec, 2018. 11 of 12 cores on biopsy. Gleason 8/9. PSA 14. Started firmigon in January x 1 month then lupron. Starter docetaxel in March thru July. After chemo PSA was .085. Had a tough time with chemo. Low blood counts all around. MO didn't start me on other meds at the time, only lupron. PSA rose slowly till 0.12 March 2020 up to 0,64 in June. July 16, 2020 started zytiga and an appointment with radiologists, oncology, today after bone scan last week. On diagnosis had 5 bone mets. Went away whit treatment I've listed. Current bone scan 2 weeks ago showed uptake to left lower trocanter and left numeral head. Also 4the posterior rib. Today radiologists offered to zap those areas and prostate bed or just the bone sites. He recommended I see cancer oncologist and radiation oncology at Duke. They called for appointment.
Update
I didnt follow up with Duke or radiologists. after a month now on zytiga with no side effects and perfect labs my PSA has gone to 0.08. Very happy with that. Will continue zytiga and 10mg prednisone. urologists wants me to start provenge. Oncologist said he wouldn't at this time. I'm leaning against it. Any comments on progress so far and suggestions? Also advice on taking or not taking Provenge?

Some say that Provenge has a synergistic effect when administered while taking Zytiga. Is the jury still out? I have no idea. I went through 3 Provenge infusions and soon after started Zytiga. I do believe that Provenge helps Zytiga stay effective for a longer period of time. However I'm not in the medical field so my Provenge/Zytiga believe is just a hunch or wishful thinking. It is a common belief that Provenge is best given while a PSA is under 2.
Thanks for your reply Tall_Allen. I'm thinking of staying the course I'm on now. Having good results so far. With out radiation do you think provenge would be suitable now? Would it be prudent to wait and monitor my progress? Just your opinion as I value your input.
To get the most benefit out of Provenge, why not use it with Xofigo or spot radiation? You only get the one opportunity, it seems a good idea to maximize its impact.
I would follow the recommendations. Knock that pc out of the park. Whatever you choose ? May it be the best for you . Let’s those at Duke see you thru this. Good day Altamontt....

Good luck in fighting this bastard. After reading your post twice, I was still confused. Who is treated your metastatic prostate cancer? Urologist? General Oncologist? Radiation Oncologist? Medical Oncologist specializing in Genitourlogic Cancer? On none of the above..... I ask because any medical doctor can prescribe Lupron, order scans, and make recommendations.......
I've been treated by my urologist an MO. Both very competent. They consult with each other and also radiation oncologist. I agree with their treatment and am considering spot radiation and provenge as suggested by Tall_Allen. My psa is lower now than ever, 0.080.
