GP wants to see me regarding Thyroid results

Hi Everybody,

I have had the following readings from my last 2 blood tests.

Dose 150mg Levothyroxine

20/11/24 TSH <0.01 (0.35 - 4.94)

20/11/24 T3 N/A (2.4 - 6.0)

20/11/24 T4 17.8 (9.0 - 19.1)

Dose reduced to 125mg Levothyroxine (asked to re-test 3 months later)

28/03/25 TSH 0.04 (0.34 - 4.94)

28/03/25 T3 4.0 (2.4 - 6.0)

28/03/25 T4 13.8 (9.0 - 19.1)

What do I need to arm myself with when I see the GP please?

How do you feel? Do you have symptoms?

What are your recent vitamin results B12, Folate, Ferritin and Vit D?

Without these answers it’s difficult to answer.

Well, I know my mood fluctuates and I have gained a few kgs since my does was reduced, and I don't sleep much (however I think my sleep pattern is not necessarily related to this)

Interestingly here are my other results that you have asked about:

B12: 1000 (200 - 900) I am not on any supplements for last 6 months. (22/11/24 was 815)

Serum Folate: 15.5 (3.0 to 20.5) I have been on Folate supplement as previous test 22/11/24 was 2.3.

Ferritin and Vit D was last measured in November 2024 and it was within range.

Do you have the Ferritin and Vit D result?

Also was your Thyroid test done 24 hours after last dose and before 9am?

Vit D: 62 (50 - 200)

Ferritin: 118 (30 to 300)

Yes thanks to this forum I made sure the test was 24 hours after last does and before 9am.

Your Vit D is too low. You either need to increase your dose or start supplementing again. I’d recommend the Better You Vit D 3000 + k2.

With regards your GP appointment, I doubt they will increase your dose with what they will see as a deficient TSH and may even push you to reduce your dose to 100mcg. I’d resist that at all costs as your FT4 could possibly do with a bit more. If you have some spare tablets, as suggested a year ago you could try taking an extra 12.5mcg every other day and then retest in 6 weeks to see if that gets you to the right spot along with the raised Vit D. Also you might want to think about testing your male hormones, although I’m not going to advise on those!

On 125mcg levo

28/03/25 T3 4.0 (2.4 - 6.0) result is 44.44% through reference range

28/03/25 T4 13.8 (9.0 - 19.1) result is 47.52% ditto

We aim to have the % age roughly approaching 75%....your numbers are low but we are all different so consider any symptoms!

There is no evidence of poor T4 to T3 conversion .....i.e. high FT4 with low FT3

However for most people higher results would suit better.....hence how do you feel on lower dose.

Have any symptoms of undermedication arisen?

If you felt better on 150mcg ask to return to that dose.

On 150mcg your FT4 was 87.12%. Did you feel better on this level?

Your GP is likely focussing on TSH which science proves is not a reliable marker. It is a pituitary, not a thyroid hormone!

It's not all about numbers!

PS.....Vit D needs to be at least 100. Take with vit K2 which directs the vit D to thebones and teeth and away from the arteries

Don't be persuaded to reduce dose if you don't agree

you could push for compromise dose of 137.5mcg daily - cutting a 25mcg in half

Or 125mcg and 150mcg on alternate days

Which brand of levothyroxine are you taking

Do you always get same brand at each prescription

I'm taking the same brand which is called Teva for a long time now to be fair. I don't know how good they are compared to other brands though.

I will suggest the compromise dose and see what they say.

Many people find Levothyroxine brands are not interchangeable.

Teva makes 12.5mcg 25mcg, 50mcg, 75mcg and 100mcg

Many patients do NOT get on well with Teva brand of Levothyroxine.

But for others Teva is the best

Teva is lactose free, but contains mannitol as a filler instead of lactose, which seems to be possible cause of problems for some people. Mannitol changes gut biome in some people

Relatively new ……Hillcross brand

This is a box, rather than a brand. 50mcg and 100mcg are Accord brand….but 25mcg is Teva brand

Lactose free brands - currently Vencamil or Teva only

Vencamil is lactose free and mannitol free. originally only available as 100mcg only, but 25mcg, 50mcg and 75mcg tablets became available Sept 2024

Prior to March 2023 Vencamil was called Aristo

Vencamil often very well tolerated/best option for many people

Other options

Mercury Pharma make 25mcg, 50mcg and 100mcg tablets 

Mercury Pharma also boxed as Eltroxin. Both often listed by company name on pharmacy database - Advanz

Accord only make 50mcg and 100mcg tablets. Accord is also boxed as Almus via Boots, 

Wockhardt is very well tolerated, but only available in 25mcg tablets. Some people remain on Wockhardt, taking their daily dose as a number of tablets

(Not yet known if all approved dosages are or will be available).

Excipients: lactose monohydrate, maize starch, croscarmellose sodium, gelatin and magnesium stearate.
