Interim blood test results

I had a full Medichecks in August but I decided to do an interim thyroid hormone test this week as I’d not been feeling too good - not bad but - anyway. I put in a request to GP but I’m on a 6 week wait for a phone call.

I’d appreciate any observations please.

Free T4 -17.2. Range 11.9-21.6.

In August, this was 21.3. Range 12-24.

Free T3 -4.37. Range 3.1-6.8. In August, this was 5.1. Range 3.1-6.8.

TSH - 1.13. Range 0.4-4.5.

In August, this was 0.423. Range 0.27-4.2.

This was London Company to NHS lab. Cheap.

I take 75 mcgs levo, Accord plus Wockhardt.

was test early morning, ideally before 8am only drinking water between waking and test and last dose levothyroxine 24 hours before test

Free T4 (fT4) 17.2 pmol/L (11.9 - 21.6) 

Ft4 only 54.6% indicates you are ready for next dose increase in Levo

If GP reluctant request “trial “ increase and retest in 3-4 months via GP

Even if we frequently start on only 50mcg, most people need to increase levothyroxine dose slowly upwards in 25mcg steps (retesting 6-8 weeks after each increase) until eventually on, or somewhere near full replacement dose (typically 1.6mcg levothyroxine per kilo of your weight per day)

Adults usually start with a dose between 50 micrograms and 100 micrograms taken once a day. This may be increased gradually over a few weeks to between 100 micrograms and 200 micrograms taken once a day.

Some people need a bit less than guidelines, some a bit more

TSH should be under 2 as an absolute maximum when on levothyroxine

If symptoms of hypothyroidism persist despite normalisation of TSH, the dose of levothyroxine can be titrated further to place the TSH in the lower part of the reference range or even slightly below (i.e., TSH: 0.1–2.0 mU/L), but avoiding TSH < 0.1 mU/L. Use of alternate day dosing of different levothyroxine strengths may be needed to achieve this (e.g., 100 mcg for 4 days; 125 mcg for 3 days weekly).

Did you test vitamin D, folate, ferritin and B12 too

Test was done according to recommendations on here. I supplement with D3 K2 oral spray and Igennus Super B-Complex, still on half a tab, occasionally Jarrow Methyl Folate. Vit C 250 mgs with bioflavonoids, chewable. Other supplements are irregular.

It’s the sleep pattern that has been the main reason for retest. Previous test in August was the full Medichecks and it was ok. D had gone down from 150 to 75.7 range 50-250 which taught me not to play fast and loose with that supplement!

TSH is going up according to this test and T3 and T4 are reducing.

I’m 73.663 kilos.

That’s quite low dose

Retest vitamin levels at next test …..2-3 months after increase in levo

Thank you. The B vits still give me the heebie- jeebies somewhat - though improving.

Last Medichecks in August had folate at 10.6 range <7.

B12 was 76.9 range >37.5.

You could take a separate folate 2-3 days a week as well
