Hello I’m back again with my 12 year old daughters updated blood results. I hope I got everything that you all recommended to get.

TSH 0.06 (0.68 - 4.53)

Free T4 14.6 (7.9 - 17.8)

Free T3 6.9 (4.0 - 6.2)

B12 266. (140 - 844)

Folate 9.5. (3.1 - 19.9)

Ferritin 21. (11 - 302)

Vitamin D 59. (50 - 125)

I would appreciate anyone who has any advice for me. Thank you so much

How much Levo is she taking? It looks like she's slightly over replaced at the moment as her FT3 is on the high side.

How are you getting on with raising her low vitamin levels, particularly B12?

Hi Jaydee she is on 50 mcg 5 days a week and 75 two days a week. I have done nothing about raising any levels cos I don’t really know how I could do this

Discuss with GP and if they wont offer supplements/injections then you need to purchase yourself.

Suggest she tries 50mcgs 7 days but run this past Endo.

Jaydee thank you so much for your recommendation s on previous post. I did read this of course but I was waiting for these repeated blood results before I decided to look into raising levels. I really appreciate your time. Thank you

This is what I would do and have done with my daughter.

Vitamin D first would be my choice, get some drops or a mouth spray from Amazon and take 4000iu a day Sept till April and test again, should be able to drop to 2000iu. Get D3 with K2, makes the calcium go into bones. V important for a young woman. We take ours in the evening.

Everyone in the UK should be on it Sept to April (NHS guidelines) your daughter needs more than most. GPs expect us to sort it out for ourselves.

2 weeks later start B12 and a week later folate (B Complex or separate - ask GP for a recommendation. Lots of B Complex recommendations here on the forum, they are not made equal so shop wisely. Thorne Basic B is good. I made a spreadsheet and will try and find the link and edit it in here. We take ours in the morning 2 hours after Levo.

2 weeks later start a ferritin supplement eg Three Arrows simply heme (again check with GP but low ferritin without anaemia is a condition in itself with same symptoms as anaemia and treatment the same) 1 a day till the bottle runs out (90 days) test towards the end of that. We take ours before tea in the late afternoon, heme absorption isn’t affected by food/tea/coffee/chocolate like iron supplements are so for most they are pretty effective and you don’t have to worry about meals. 🌱

was test done early morning and last dose Levo 24 hours before test

Was last dose day before a 50mcg or 75mcg dose

Her ferritin is deficient she needs full iron panel test by GP

Likely to need iron supplements

What’s her diet like

Look at increasing iron rich foods in her diet

Red meat regularly. Liver or liver pate one a week

Etc etc

B12, folate and vitamin D all need some improvement

Hello, agree with the previous posts above and sadly my daughter has a similar profile and Hashimotos.

Did your daughter have her medication before her blood test at all?

That aside, her FT3 looks good which is the active hormone and it’s hard to increase Levo despite FT4 being low if the FT3 then goes up further. (I also note that sometimes, if FT3 is very high, it can indicate an autonomous thyroid nodule throwing out hormone so she should really have her thyroid scanned for that and/or it can be down to an iodine deficiency but this is tricky to work out).

What are your daughters symptoms? Does she follow a ‘normal’ diet or is she vegan or anything else as her ferritin and B12 are low in addition to the other levels.

I think you’ll find a lot of your daughters symptoms are down to low nutrient status as opposed to the thyroid given her FT3 appears good.

Action:

1) Take Vit D+K2 at high levels and retest. A Norwegian study showed that 30mins full body sun exposure creates 10,000 units of D, so you can see we’re all rather deficient. If you have Facebook, Vitamin D wellness is a nice group

2) Ferritin - low, the iron store and anything under 30 according to NICE guidelines in UK needs treating by the GP for iron deficiency. The Iron Protocol on Facebook again another nice group. Three Arrows iron is heme so more easily absorbed but also has a good webpage for information.

3) I would ask GP to run some further B12 tests before supplementing as her level is considered deficient. If she doesn’t eat B12 products eg meat, eggs etc due to diet then this explains the level and supplements may work. Otherwise, if she eats well, I would say she has a big issue with B12 processing. Further tests would include: MMA, homocysteine, intrinsic factor antibodies. There are other forums on healthunlocked for B12 but b12info.com is great source of info. Hashimotos and B12 deficiency go hand in hand but deficient B12 would make her feel quite awful. Do not supplement with any B12 until further testing complete assuming if not diet related as this will skew the results. Your daughter may well need B12 injections. Has she had a FBC?

4) Once any B12 issues further investigated, you need to start folate before any B12 injections. Folic acid isn’t always tolerated or absorbed as it’s a non bio available form of folate so if that doesn’t help, she can try folinic acid or methylfolate form. Again, B12 Wake Up on Facebook very helpful forum along with the webpage above. Both Folate and B12 work together but supplementing either of those can skew a blood test and delay the correct treatment.

Get treating Vit D asap, then iron ASAP whilst investigating B12.

I think you meant the reverse of this

B12 start BEFORE folate

Note that improving folate when B12 is very low is not a good idea. Taking folate before B12 is good enough can lead to severe neurological problems.
