I am a 59 year old, 74Kg male, with no other comorbidities except mild lifelong well controlled Asthma.

The tests on 10/01/2024 where all taken using the guidance given here, of no levy for 24 hours prior and no food/drink ETC before the test. The previous tests may not have been in full compliance with this guidance. I take no supplements.

My latest results are.

18/12/23 by MonitorMyHealth (NHS Lab)

Range

FT3 4.2 (3.1 – 6.8 pmol/L)

FT4 20.5 (10.5 – 22 pmol/L)

TSH 2.78 (0.27 – 4.2 mIU/L)

Vit D 61 nmol/L

HB 165 g/L

Ferritin 120 ug/L

Active B12 84 pmol/L

Folate 6.9 ug/L

TPO 9 IU/mL

GP. MMH Range

FT3 4.4. (3.1 – 6.9)

FT4 20.5 19.2. (10.5 – 22 pmol/L)

TSH 2.77 2.98 (0.27 – 4.2 mIU/L)

Ferritin 115 ug/L

Vit B12, Serum 343 ng/L

Folate 6.9 ug/L

My main signs & symptoms since around November have been:

• Tinnitus all waking hours.

• constant mild headache

• Constant muscle aches, joint aches

• Constant brain fog, unable to focus.

• Constant mild stomach-ache with occasional waves of nausea.

• Feeling cold but I suddenly overheat on exertion and feel quite ill.

• Low Temp c36.8, Resting pulse 55BPM, mild constipation.

Losing the outer edges of my eyebrows

I never suffered any of the above before suffering Subacute Thyroiditis in Sept 2021, with the exception of occasional tinnitus, when I was off colour.

I volunteer at a steam museum one day a week, the next day I am completely wiped out and unable to function.

So far I felt my best in June 2023 with none of the above signs and symptoms on 75mcg of Levo.

22/06/22 by MonitorMyHealth

FT3 3.9 (3.1 – 6.9)

FT4 21.4 (12.0 – 22.0)

TSH 1.56 (0.27 – 4.2)

In July in agreement with my GP I undertook a controlled T4 reduction with not good results. reduced from 75mcg Levo to 25mcg before resuming to 75mcg on 15/11/23. I am currently on 50mcg of Accord (every month) & 25mcg of Wockhardt (last 2 months) per day, taken at night.

15/11/23022 by MonitorMyHealth

FT3 4 (3.1 – 6.9)

FT4 16.5 (12.0 – 22.0)

TSH 12.9 (0.27 – 4.2)

Strangely my T3 increased slightly but I felt terrible, and I still do. All the other bloods my GP did on the 10/01/2024 are all normal except for

Basophil count (42L..) 0.1 10^9/L [0.02 - 0.1] - Above high reference limit

Which is just at top of range and can indicate Hypothyroidism amongst many other things.

I have a GP appt soon and as always appreciate all your comments and advice as to the way forward.

Push GP too increase dose Levo

Initially perhaps 75mcg and 100mcg alternate days

But after few weeks up to 100mcg daily

Do you normally take levothyroxine waking or bedtime

Meanwhile

Vitamin levels need lot of improvement

Vit B12, Serum 343 ng/L

Folate 6.9 ug/L

Low B12 symptoms

b12deficiency.info/signs-an...

methyl-life.com/blogs/defic...

With serum B12 result below 500, (Or active B12 below 70) recommended to be taking a separate B12 supplement and a week later add a separate vitamin B Complex

Then once your serum B12 is over 500 (or Active B12 level has reached 70), you may be able to reduce then stop the B12 and just carry on with the B Complex.

If Vegetarian or vegan likely to need ongoing separate B12 few times a week

Interesting that in this research B12 below 400 is considered inadequate

Low folate

supplementing a good quality daily vitamin B complex, one with folate in (not folic acid)

This can help keep all B vitamins in balance and will help improve B12 levels too

Difference between folate and folic acid

healthline.com/nutrition/fo...

B vitamins best taken after breakfast

Thorne Basic B recommended vitamin B complex that contains folate, but they are large capsules. (You can tip powder out if can’t swallow capsule)

Thorne currently difficult to find at reasonable price, should be around £20-£25. iherb.com often have in stock. Or try ebay

Igennus B complex popular option too. Nice small tablets. Most people only find they need one per day. But a few people find it’s not high enough dose

IMPORTANT......If you are taking vitamin B complex, or any supplements containing biotin, remember to stop these 7 days before ALL BLOOD TESTS , as biotin can falsely affect test results

endo.confex.com/endo/2016en...

endocrinenews.endocrine.org...

In week before blood test, when you stop vitamin B complex, you might want to consider taking a separate folate supplement (eg Jarrow methyl folate 400mcg) and continue separate B12

Thank you for your prompt and detailed response.

I take my Levo around 22:00, I followed your advise of moving the evening dose to the morning preceding the test day and then dose immediately after the blood draw ETC.

Vit D 61 nmol/L

GP will often only prescribe to bring vitamin D levels to 50nmol.

Some areas will prescribe to bring levels to 75nmol or even 80nmol

leedsformulary.nhs.uk/docs/...

GP should advise on self supplementing if over 50nmol, but under 75nmol (but they rarely do)

mm.wirral.nhs.uk/document_u...

But improving to around 100nmol by self supplementing may be better

Once you Improve level, very likely you will need on going maintenance dose to keep it there.

only start one supplement at a time

Wait at least 10-14 days to assess before adding another

Meanwhile also improve low ferritin by increasing iron rich foods in your diet

Constant mild stomach-ache with occasional waves of nausea

Low vitamin levels are result of lower stomach acid when hypo

Lower stomach acid also more common as we get older

I am a huge supplement fan. B12's, D and C along with a dozen others, including BLACK SEED (Nigella sativa). I can not take thyroid synthetics so had to design my own treatment for my thyroid.

If you are reliant on levothyroxine can't help much as I was mis diagnosed and put on that and it almost killed me. It took me months to get my body balanced after the damage that it did to me. Just me. I was allergic to that medicine apparently. I digress. My vitamin and mineral levels were all over the place after only a few days and I had to increase all supplements and vitamin to even try to get out of bed.

I use powder supplements mixed with water in a jar ... drink the 1/3 of the jar three times a day. Start slowly with low amounts of ALL the supplements and increase over time to what is your optimum amount. Example ... some people need more B12, some more MSM, iodine, etc. Do your research and don't be afraid of asking questions. There are some excellent people and sites to assist with supplements and vitamins on the web.

Black seed is what I used for my thyroid issues. I put the seeds (or oil) in yoghurt in the morning arvo and or evening with my b12 powder (above what is in the jar). Age and pain zaps my energy and Queen Bee honey and 12s gets me going again.

Just remember if you have had any XRAYS dental work, etc it does effect your thyroid and you will get a horrible reading if bloods are taken before your body can bounce back from the RFs emitted from those.

best to you in the new year.
