I've had my first ever Medichecks Advanced Thyroid Test and would really appreciate your views on next steps. The TSH result would indicate I need to increase levo, also I'm concerned at high CRP and antibodies! Plus I don't think I convert T4 to T3 very well. Your thoughts are very much welcomed and valued.
I followed proper protocol on timing and pausing supplements, I am currently on 125mcg levothyroxine a day (6ft 105kg female) and have been taking pink tribe methylated b complex, vitamin d3 8kiu, magnesium glycinate, plus natto powder for vit K2.
My results are as follows:
TSH 6.09 (0.27-4.2mlU/L)
FT3 4.4 (3.1-6.8pmol/L)
Ft4 17.2(12-22pmol/L)
Thyroglobulin antibodies 629 (0-115klU/L)
Thyroid Peroxidase Antibodies <9 (0-34 kIU/L)
CRP HS 3.75 (<3 mg/L)
Ferritin 109 (30-150 ug/L)
Folate - Serum 25.5 (8.83-60.8 nmol/L)
Vitamin B12 - Active >150 (37.5-188pmol/L)
Vitamin D 120 (50-250nmol/L)
Thank you for reading to the end

Hi, yes you certainly need an increase to bring your free's up, your conversion doesn't look too bad
TSH 6.09 mIU/L (0.27 - 4.2) 148.1%
Free T4 (fT4) 17.2 pmol/L (12 - 22) 52.0%
Free T3 (fT3) 4.4 pmol/L (3.1 - 6.8) 35.1%
T4:T3 Ratio 3.909
As a rough rule of thumb 1.6mcg per kilo in body weight so not surprising that an increase is needed 🤗
I wouldn't be overly concerned that your antibodies are high as that is just a reflection of the damage already done to your thyroid, CRP is only slightly up
Actually, no, the antibodies are not just a reflection of damamge already done. They are an indication that the cause of your hypo is Autoimmune Thyroiditis - aka Hashi's - and therefore that further damamge will be done. The disease will completely destroy your thyroid. But, as you're already on thyroid hormone replacement, that won't make much difference to you.
What will make a difference is the probability of Hashi's 'hyper' swings, when levels will become temporarily well over-range, and your doctor will possibly accuse you of 'abusing' your thyroid hormone, and want to slash your dose. Because they know nothing about Hashi's! So, it's as well if you know.
Antibodies fluctuate all the time, but tend to be highest during/just after an immune system attack on the thyroid, when they come along to clean up the mess in the blood. So, they are nothing to be worried about, they are your friends.
Thank you greygoose, I love the idea of the antibodies being my little friends - makes me feel much more comfortable having elevated levels I had a bad cold the week before my test which may or may not have been covid (i did a lateral flow test which was negative but anecdotally I'm not sure this always means it wasn't covid) anyway i was off my feet for a couple of days and in a fug for about 2 more. About 4 months ago I got a cough which was coming from my thyroid area (not lungs) - we chatted about this at the time if i recall - GP sent me for a chest xray and thyroid ultrasound (results - chest clear but thyroid is small and heterogenous so fits with hashi) - i now feel very sorry for my poor shrivelled attacked thyroid! I've waited until now to redo bloods as i embarked on a supplements regime and wanted to give it time to work. Seems to have worked well except for TSH going up and me getting a bit fatter lol so looks like i need to get a levo dose increase!
