Hi,I wonder if someone could advise please recent tests show

T4 22.4 pool/L (12-22) noted as high

TSH level 2.30 miu/L (0.27-4.2)

I have been having lots of hot sweats and brain fog,Conspitation and Anxiety.

My last TSH was below 1 and i felt fine do I need an increase or decrease in Levothyroxine

Sorry no T3 tested blood taken first thing fasting but Levo taken night before as didn’t have a chance to not take it night before.

I have a telephone appointment with GP on Wednesday so would like some advice please.

I also had a Platelet count of 504 (150-450) Marked High

Basophil count 0,13 (0.01-0.1) Marked High

How much levothyroxine are you taking and which brand

How long on this dose

Recommended that all thyroid blood tests early morning, ideally just before 9am, only drink water between waking and test and last dose levothyroxine 24 hours before test

This gives highest TSH, lowest FT4 and most consistent results. (Patient to patient tip)

Is this how you did your test?

No vitamin D or ferritin results?

Folate on low side

What vitamin supplements are you taking

Previous post 4 years ago ferritin was deficient

I’m taking 100mg of levothyroxine (Accord) I have been taking this for a couple of years now.

Last time my Vitamin D was tested I was over range,My iron levels are always on the low side but unable to take supplements although I do try and have iron rich foods.

I take theMagnesium supplement Floradex liquid formula 20ml.

Test was done before 9am fasting water only but levothyroxine taken at 10pm the previous evening.

So Ft4 is falsely high - ignore it

Last dose levothyroxine should be 24 hours before test

next test

If you normally take levothyroxine at bedtime/in night ...adjust timings as follows prior to blood test

If testing Monday morning, delay Saturday evening dose levothyroxine until Sunday morning. Delay Sunday evening dose levothyroxine until after blood test on Monday morning. Take Monday evening dose levothyroxine as per normal

Request GP retest iron and ferritin levels

Okay I will ask the GP to test iron and ferritin levels I’m just worried in case she tries to reduce my medication,I’m new to the surgery and she doesn’t know me well.

I don't have any advice, sorry. But I would be interested in what your GP has to say about your high platelets because I have the same problem.

Iron-deficiency anemia and hemolytic anemia can cause thrombocytosis.

Hi I’ve always had high platelet count usually around 650 ish also high basophil & eosinophils count. GP’s response has always been “we know yours is always high”.

I think they said inflammation due to RA. I’m interested to know how you get on.

Well she wasn’t bothered about the high Basophil or Platelets,I am having more Blood tests and she will keep an eye on them but thinks I may need an increase in Thyroxine and has agreed to test my iron and Ferritin levels ,She agreed my T4 result were probably a false high.

So still waiting until after next results but at least she listened to me.

Thanks. It’s strange they don’t seem bothered they are raised don’t you think? I’m glad they are doing more blood tests though. And it’s good to be listened to. Everything is a waiting game unfortunately

Thank you Otto 11,yes it is strange but hopefully in the long run we will get to the bottom of it and if I can get an increase of my Levothyroxine I think I will feel much better.

Suggest you improve Low folate and B12

Low B12 symptoms

With serum B12 result below 500, (Or active B12 below 70) recommended to be taking a separate B12 supplement and a week later add a separate vitamin B Complex 

Then once your serum B12 is over 500 (or Active B12 level has reached 70), you may be able to reduce then stop the B12 and just carry on with the B Complex.

If Vegetarian or vegan likely to need ongoing separate B12 few times a week

Low folate

supplementing a good quality daily vitamin B complex, one with folate in (not folic acid)

This can help keep all B vitamins in balance and will help improve B12 levels too

B vitamins best taken after breakfast

Thorne Basic B recommended vitamin B complex that contains folate, but they are large capsules. (You can tip powder out if can’t swallow capsule)

Thorne currently difficult to find at reasonable price, should be around £20-£25. iherb.com often have in stock. Or try ebay

Igennus B complex popular option too. Nice small tablets. Most people only find they need one per day. But a few people find it’s not high enough dose

IMPORTANT......If you are taking vitamin B complex, or any supplements containing biotin, remember to stop these 7 days before ALL BLOOD TESTS , as biotin can falsely affect test results

In week before blood test, when you stop vitamin B complex, you might want to consider taking a separate folate supplement (eg Jarrow methyl folate 400mcg) and continue separate B12
