Her free hormone results are terribly low. 75mcgs Levo is also a fairly low dose so she needs an increase in Levo to raise her free results. If her TSH got higher then likely free numbers would fall.
Regarding iron/ferritin, its usually recommended to run a full iron panel before beginning an iron supplement. Its possible to have low ferritin and high iron and having too much iron isn't a good thing.
For low ferritin its suggested to increase iron rich foods in the diet such as liver/pate a few times a week, increase red meat.
Vitamin D should be around 100 - 150. Buy one that includes vit K2 to help it go to your bones. Some are available in oil or you can take it with an oily meal for better absorption. Would her frees improve if TSH gets a little higher?
That's how it works for someone with a normally functioning thyroid. But, your sister is hypo and taking thyroid hormone replacement, so it no-longer works like that.
To get her TSH to rise, she would have to reduce her dose of levo. That would mean that her FT4/3 levels would drop.
TSH - Thyroid Stimulating Hormone - has no effect on thyroid hormone that you take orally. It only increases levels by stimulating the thyroid to make more hormone. However, her thyroid can no-longer respond to that stimulus, which is why she need to take oral hormone to replace it.
What she needs to do to increase her FT4/3 levels is to increase her dose of levo and ignore the TSH completely.
