Blood result advice please

Hi all could I trouble you for some advice on my blood results, please? I'm hypothyroid and currently on 75mcg x 5 days a week and 100mcg x 2 days a week via endocrinologist. It is been a very long road of severe symptoms but definitely feeling a little brighter over last month or so.

Latest below

TSH 0.23 mIU/L (0.27 - 4.20) -1.0%Free T4 (fT4) 19.4 pmol/L (12 - 22) 74.0%

Free T3 (fT3) 3.7 pmol/L (3.10 - 6.80) 16.2%

T4:T3 Ratio 5.243 

Folate - Serum 20 ug/L (3.9 - 26.80) 70.3%

Vitamin B12 1269 nmol/L (197 - 771) 186.8%

Ferritin 41.9 ug/L (13 - 150) 21.1%

Serum iron 16.6 Umol (10 - 30) 33.0%

Serum transferrin 2.63 Gl (2.0 - 3.60) 39.4%

Could anyone advise on T3. It has actually dropped. It was 4 (3.1-6.8) in Jan. What should I be aiming for? Does this dhow I'm not veneering well. Could this be why I'm still symptomatic.

What about TSH is that significant. It was 0.13 in Jan so endocrinologist increased levothyroxine to 100 x twice a week.

I take a b compex. Should I come off it with thst b12 result?

Ferritin seems low.Any thoughts?

Looking through previous posts

Do you know if cause of your hypothyroidism is autoimmune

Saw you are Negative TPO antibodies

Need TG antibodies tested.

If not tested yet ………You will need to test privately as NHS only tests TG antibodies if TPO antibodies are positive

Significant minority of autoimmune thyroid patients only have high TG antibodies

Blood test should always be done early morning, ideally just before 9am and last dose levothyroxine 24 hours before test

Stopping vitamin B complex 5-7 days before test as contains biotin and biotin can falsely affect test result

Is this how you did this test

are you on strictly gluten free diet and/or dairy free diet

Anti thyroglobulin antibodies were tested last year and were : 21 (<115); TPO was <9 (<34). I had a thyroid scan and was told 'small volume' thyroid and otherwise OK. Not sure what that means.

I have not been told what type of hypothyroidism I have, but I have had such severe symptoms and had a hard time on levothyroxine, I assumed it was Hashimotos.

I followed your advice at last post and have gone wheat free (3 weeks now). I had food sensitivity testing last year with Blue Horizon, but everything.

I am trying seed/grain free as well over last few weeks, as even gluten free replacements like oats were causing symptoms). Has made a noticeable difference in bloating and eye bags.

Blood test was done 8.30 am- no levo dose taken before & off B vitamins 5 days before.

Most likely Ord’s thyroiditis

About 90% of primary hypothyroidism is autoimmune thyroid disease, usually diagnosed by high thyroid antibodies 

Autoimmune thyroid disease with goitre is Hashimoto’s

Autoimmune thyroid disease without goitre is Ord’s thyroiditis. 

Both are autoimmune and generally called Hashimoto’s.

20% of autoimmune thyroid patients never have high thyroid antibodies

your FT3 is very low but your FT4 is good. Are you still going through the process of increasing Levo? You might have room for another increase to see if FT3 improves. If it doesn’t, maybe your endo will prescribe some FT3? If he does, read on here about how to take it (low and slow).

Maybe SeasideSusie can advise on your high B12?

How much B12 in your B Complex?

I have this same situation, folate needs supplementing but B12 is already high so goes way over range. If it's the supplement causing the high B12 there should be no need to worry, if it's high without supplements that's when it can be caused by a medical problem and needs investigating.
