Help with test results and ultrasound scan

A big thank you in anticipation of the responses you are all amazing.Since my diagnosis hypothyroidism early 2022 it has been a rollercoaster. High antibodies drove a complete overhaul of my lifestyle which included thyroid friendly diet, no gluten, dairy and processed foods. Limit alcohol. I have supplemented with vit d and B12 and liver weekly.

I decide to have an ultrasoumd scan just to understand a little more about the state of my thyroid. This happened 29th Dec 2022.

Findings;

The right and left lobes of the thyroid appear normal in shape, size and echopattern.

Normal vascularity seen in th gland on color Doppler.

No retrosternal extension seen.

No nodules seen.

Visualised regional lymph nodes appear morphologically normal.

Conclusion;

Normal ultrasound scan of the thyroid, no nodules seen.

I have had my medichecks advance thyroid results delivered today.

CRP HS 0.66 (<3)

Ferratin 60 (30-150)

Folate 35 (8.83-60.8)

B12 140 (37.5-188)

Vitamin D 90 (50-250)

TSH 2.15 (0.27-4.2)

Free T3 4.1 (3.1-6.8)

Free T4 18.8 (12-22)

Thyroglobulin 34.3 (0-115)

Thyroid peroxidase antibodies 70.8 (0-34)

I feel the best I have felt in a long time. Even started circuit training. However I have noticed a little more brain fog creeping in.

Compared to the test results in oct 22, i have improved b12 result, dipped a little with vit d, ferritin has increase significantly.

My thyroid peroxidase is continuing to drop.

I have been following a low fodmaps diet for 8 weeks to help improve my gut health, i will shortly move to a Mediterranean style eating plan.

Bloods were taken 9.15 due to delay by nurse.

Stopped biotin and b's as advised.

Last levothyroxine taken previous day at 7.30 am.

I am still only on 50mcg.

What are your thoughts?

FT4: 18.8 pmol/l (Range 12 - 22) 68.00%

FT3: 4.1 pmol/l (Range 3.1 - 6.8) 27.03%

Plus TSH is over 1

Suggests you are likely to need next dose increase in levothyroxine soon

Perhaps increase to 75mcg and 50mcg alternate days initially

And then to 75mcg daily

Retest 6-8 weeks after each dose change or brand change in levothyroxine

Which brand of levothyroxine are you currently taking

I have always been on accord.Do you have any observations on the scan findings?

Well you have high thyroid antibodies, so it is autoimmune

Probably not Ord’s ……or it’s just too early to say as no obvious damage to be seen

Accord don’t make 25mcg tablets

Request increase in number of 50mcg tablets and cut in half to get 25mcg
