Would really appreciate help and opinions with lab results

I have complained of lack of energy, excessive tiredness, breathlessness and pain everywhere for the last 17 years . I was Diagnosed with Osteoarthritis , Fibromyalgia and a Vit D deficiency in July 2019 ( My partner passed away suddenly 2 Oct 2019 ) I had to start chest compression's until the paramedics arrived and they made me call time when they said after nearly 2 hours that they couldn't revive him. My health has continued to slide down the slippery ladder since 2019. I run out of energy and cant walk any further as I cant even lift my legs.

(Tests dated 31 Dec 2021)

TSH .. below range ... 0.13 mlU/L ( 0.27 - 4.2 )

T4 19.6 pmol/L ( 12 - 22 )

Vit D .. below range 58.7 nmol/L ( 75 )

Diagnosed with High BP , High glucose levels , High Cholesterol

Daily Levo dose of 75 (100 Wed and Sunday ) ...... Levo reduced as per GP by 25 from 100 to 75 on Wed and Sunday.

(Tests dated 22 July 2022)

TSH .. below range .. 0.13mlU/L ( 0.27 - 4.2 )

T4 .. 20 pmol/ l ( 12 - 22.0

August 2022 diagnosed with dependant oedema and a systolic heart murmer ... waiting to see cardiologist , asked if I was tired and breathless !! and was told by paramedic filling in at the GP surgery that my heart is struggling to send enough oxygen around my body.

(Tests dated 17 August 2022)

Vit D 2 ? 73.6 nmol/L ( 75 )

Folate level (42U5 ) .. 4.6 ug/L ( 3.9 -26.8 )

B12 ( XE2pf ) 355 ng/L ( 197.0 - 771.0 )

Levo reduced again by 25 on the Sunday making 75 taken everyday now.

Tests dated 14th September 2020 ( after threatening to have private blood tests )

T3 3.2pmol/L ( 3.1 - 6.8 )

Does anyone know if there is anything I can improve on in the above results ? ..... blood results all ok no action needed according to the text message I received.

I have been taking 10,000ui of Vit D 3 everyday since March 2020 since my vit D was found to be deficient in 2019 . I took 60,000 ui once a week for 8 weeks then a maintenance dose of 10,000ui but it still drops in winter even though I use a light box as well . I have now increased to 20,000 ui as per my Osteopath ... I never had a Vit D test done even though I was diagnosed by the GP with SAD in December 2000 ? and I had taken 6 h&b D3 in winter and 3 h&b in summer for 20 years. Too tired to argue anymore.

So sorry you are struggling. Your folate is far too low, should be around 20. Do you have a ferritin result?

You really need to get a full thyroid test with the FT3 result in it. If GP won't do it (unlikely) then get one from Medichecks. Thyoid UK have a discount code on their website.

Thank you for responding .. I have asked the nurse if she would draw the blood if I got a private test done .. she said yes as long as I have all the files , paperwork etc; that she needed.

So sorry for the loss of your partner is such stressful circumstances

And subsequent extremely poor care by GP

Your low Ft3 in Sept 2020 suggests under medicated and poor conversion of Ft4 (levothyroxine) to Ft3 (active hormone )

Do you always get thyroid tested early morning, ideally just before 9am and last dose levothyroxine 24 hours before test

Do you always get same brand levothyroxine at each prescription

Which brand of levothyroxine

Request GP test FULL iron panel test for anaemia including ferritin

Other Vitamin levels are far too low

Vit D 2 ? 73.6 nmol/L ( 75 )

B12 ( XE2pf ) 355 ng/L ( 197.0 - 771.0 )

Optimal vitamin levels are

Vitamin D at least over 80nmol and around 100nmol maybe better

B12 at least over 500

Folate at least half way through range

Only add one supplement at a time and then wait at least 2 weeks before adding another

6-8 weeks after adding last supplement retest thyroid levels

Will do in-depth replies on vitamins as separate replies

Thank you for responding and for all the advise and info .. I have a lot of reading to do .

I don't know if I have Hashimoto's ? I have never been told .

I have had problems all my life .. just told I was lazy at school aged 6 because I had no energy to run around like everyone else , it took me much longer than everyone else to absorb information , I was forever freezing cold and eventually had to have an electric over blanket when I was 12 , my temp on average is 34 deg sometimes 35deg , if I get cold I can't warm myself up and I get stiff ? , really bad periods and pain from when I was 10 and so it went on but I didn't know any difference ...

Until I saw a gynecologist in S Africa in 1978 and he asked me '' how long has your neck looked like that ? '' after blood tests I was diagnosed hypothyroid .

I have seen an Endo here , he said if I had been given growth hormones when I was young I wouldn't have any of the problems I have now and have had all my life, but he never mentioned Hashimoto's so I don't know.

I’m so sorry for your loss, I can only imagine how you must be feeling. I’m not sure if someone else answered this but you can request thyroid antibodies be tested. TPOab, and ThyroGlobulin. I’m also not sure your GP keeps lowering your medication as it looks like you need more. I also agree with the above advice that your vD is low and so is B12. I hope that your hear situation is sorted out and that your Drs take better care of you. I too have Fibromyalgia among many other things and it can be hard to tell what is causing what. Would be good to get Iron labs with Ferritin and see what your storage amount is too. Both Thyroid, heart, and Iron issues can cause what you’re experiencing and it’s important to know.

cein , do you think your thyroid levels are changing up? I took me a minute to realize you’re going low, now hypo low (above range) which is hypo but seemingly hyper- low (to the left side). This may be why they are lowering your dose. I’m bit a dr but told Hashimoto’s can do this (going back and forth). Has it done this before? I’m actually post Graves’ disease and have both TSI antibodies and TPO antibodies, and my levels where always on the left until my thyroid burned out in 2016. Might be good to check for Graves antibodies too. I got Graves after the birth of my daughter; do you feel heart palpitations at all and restlessness? Maybe check Graves symptoms too, just in case. Just a thought, sorry for first post- fibro brain and coming out of a migraine

vitamin D

Ditch the H & B vitamin D and get a good quality vitamin D

GP will often only prescribe to bring vitamin D levels to 50nmol.

Some areas will prescribe to bring levels to 75nmol or even 80nmol

GP should advise on self supplementing if over 50nmol, but under 75nmol (but they rarely do)

But with Hashimoto’s, improving to around 80nmol or 100nmol by self supplementing may be better

Recommend adding B12 first …..then after 2 weeks start on daily vitamin B complex

With serum B12 result below 500, (Or active B12 below 70) recommended to be taking a B12 supplement as well as a B Complex (to balance all the B vitamins) initially for first 2-4 months.

once your serum B12 is over 500 (or Active B12 level has reached 70), stop the B12 and just carry on with the B Complex.

Do you have autoimmune thyroid disease also called Hashimoto’s usually diagnosed by high thyroid antibodies

About 90% of primary hypothyroidism is autoimmune thyroid disease, usually diagnosed by high thyroid antibodies 

Autoimmune thyroid disease with goitre is Hashimoto’s

Autoimmune thyroid disease without goitre is Ord’s thyroiditis. 

Both are autoimmune and generally called Hashimoto’s.

Low vitamin levels are extremely common when hypothyroid, especially with autoimmune thyroid disease (Hashimoto’s or Ord’s thyroiditis)

20% of autoimmune thyroid patients never have high thyroid antibodies and ultrasound scan of thyroid can get diagnosis 

In U.K. medics hardly ever refer to autoimmune thyroid disease as Hashimoto’s (or Ord’s thyroiditis)

Recommended on here that all thyroid blood tests early morning, ideally just before 9am and last dose levothyroxine 24 hours before test 

This gives highest TSH, lowest FT4 and most consistent results. (Patient to patient tip)

Vitamin A (25,000 units / day) has been shown to increase conversion of T4 to T3. It's important to know that retinyl palmitate was used as the supplement versus beta carotene, which is found in carrots and can't be utilized efficiently by the liver in those with low thyroid function, causing an Oompa Loompa effect on the skin color. Check the label before buying.
