Hi,

I have recently had my (fairly) regular private blood test done, I'm posting a photo of my results from January 2020, October 2020, and July 2021. Ferritin result which isn't visible in the photo is around 90. Antibodies are normal.

I currently take 125mcg of levothyroxine and 12.5mcg of liothyronine, levo and half of lio around 7am, second half of lio around 2pm. I take an array of supplements (all listed I'm my previous posts, dosages unchanged). I have noticed a serious drop in energy levels, increased hair loss etc. over the past few weeks. There seems to be a drop in my T4.

Any advice on what route to take?

Is the right hand photo the July one?

Last dose of Levo should be 24 hours before test.

Last dose of T3 should be 8-12 hours before test.

What was the time of your test, I imagine last dose of T3 was more than 12 hours before.

Yes, the right hand photo is last month's result.

Test was done at 9:30am, last levothyroxine 9am previous day, T3 was 8pm night before.

OK, you didn't mention the times of last doses in your post so I assumed they were taken at the times you did mention.

I would say that you are currently undermedicated. I take a levo/T3 combination and your FT4 level would have me feeling very unwell. I need both FT4 and FT3 fairly well balanced around 60-70% through range although everyone is different.

Your FT3 is low in range at just 37.84% through range whereas with most people on combination hormone replacement you would expect to see FT3 in the upper part of it's range. Your FT4 is only 23% through it's range.

How did you feel when your FT4 was 18.3 and your FT3 was 4.88?

I definitely felt better, the current slump is significantly worse than any I've had over the past year. But I've also had the second dose of the covid vaccine 2 weeks ago and the first one (in May) knocked me out in a similar fashion back then, so I wasn't paying too much attention to my increased symptoms until I saw the results...

Maybe increase thyroid meds then.

If you felt better with a higher FT4 then maybe increase Levo first, that should also increase your FT3. Retest in 6-8 weeks (or leave however long it takes for your levels to stabilise and you to feel the full benefit of a dose change). Then if necessary another increase and decide if increasing Levo or T3 would be the better option. Don't change both at the same time or you wont know which has made a difference.

Personally I would take it slowly (basing that on personal experience) and I would raise Levo by 12.5mcg. You could try 25mcg but it's up to you. Retest in 6-8 weeks or longer if you know you need longer for your levels to settle after a dose change.

Same brand T3

Have you tried strictly gluten free diet

Levothyroxine is always Accord for the 100mcg and Teva for 25mcg, that hasn't changed in 2 or 3 years (I get it sent by Pharmacy2U), T3 I used to get from Czech Republic was Cynomel by Sanofi, after supply issues last year I had to swap to Greece bought T3 made by Uni-Pharma (that was roughly 9 months ago, didn't notice any adverse effects). I haven't tried gluten-free diet.
